Reporting Template
Elementary School Educational Technology Assessments
We designed this worksheet form for optional use only as an efficient way to capture data about the Educational Technology assessments.
School districts that use the OSPI-developed assessments for Educational Technology at the elementary, middle, and high school levels, must report
that information to OSPI (RCW 28A.655.075). We provide this worksheet form to make data entry easy. Note that we do not collect information about
individual teachers and schools.
Although not compulsory on the report, we also hope to gather information about other assessment strategies teachers used to measure student
learning in this subject. If you decide to use this form, please share it with the person in your district who handles reporting for OSPI.
To complete this worksheet:
1.
Fill in the total (unduplicated) number of students who completed one or more assessment for Educational Technology.
2.
Fill in the number of assessments completed for specific OSPI-developed assessments for Educational Technology.
3.
List any alternative assessment(s) you used to assess the Educational Technology standards.
