APPLICATION FOR INSTITUTIONAL ENDORSEMENT
Students seeking endorsement from Iowa State University will submit the following documents to Dana
Schumacher, 2130 Jischke Honors Building, by 5:00 p.m., April 6. The Selection Committee will review
draft applications to select candidates for follow-up interviews. Following interviews, the Committee will
select endorsed candidates.


A RESUME OR OTHER LISTING OF YOUR COLLEGE ACTIVITIES AND HONORS: awards,
scholarships, employment, research, leadership, service, athletic record, publications, etc. This
should be no more than two pages in length.

A COPY OF YOUR CURRENT DEGREE AUDIT OR TRANSCRIPT (either can be printed from Access
Plus)

A LIST OF 5 – 8 REFERENCES: listing name, title, organization and the context in which the
person knows you. This list should include those writing the required two letters of
recommendation.
