Transition Teacher
Timeline of Responsibilities
All Year Long
 Get a list of current senior and junior ESE students from your data entry
operator
 Write a letter of introduction to be sent home to students and parents
 Organize action plan, transition interview forms, and Summary of
Performance (SOP) into file folders for each senior
 Attend T/IEP meetings for at-risk juniors and seniors
 Make sure juniors and seniors planning to attend college take SAT and
ACT
 Make agency referrals and follow-up (ex. VR, APD)
 Meet with School Counselor, Career Specialist, and Job
Placement/Transition Specialist and provide copies of action plans
 Organize and issue Transfer of Rights monthly

Notify case managers that you will be meeting with juniors and seniors
Develop a database of the students you will be working with
Conduct interviews and write action plans with seniors then juniors as time permits
Send letter of introduction to parents of all seniors and copy of action plan via mail
Follow-up with parents by phone to review action plan
Distribute Youth Leadership Forum applications to appropriate juniors and seniors
(Obtain from your School Counselor)
 Send letter of explanation and MIS Form #703, found in Planmaker, home to the
parents to be signed and returned (Retain a copy for your records)
September
 Continue to notify case managers that you will be meeting with juniors and seniors
 Continue to conduct interviews and write action plans with seniors then juniors as
time permits
 Continue sending letters of introduction and copy of action plans via mail
 Continue following up with parents by phone to review action plan

Check senior grades
Meet with any senior at-risk of not graduating due to credit or GPA issues
Meet with seniors graduating in December to review the Summary Of Performance
(SOP). Provide student a copy of the SOP and file the SOP in the red cum folder on
top of the current T/IEP
January
 Check for grade level mid-year changes and conduct interview and action plan for
students that are now considered seniors

 Send home change of placement form (MIS 797) for graduating seniors and students
who are aging out
 Send out Transfer of Rights for June and July birthdays (juniors and seniors)
