It is one of the largest cardiovascular centers in Russia with capacity for 167 patients (including 17 children). About the Project The Federal Center for Cardiovascular Surgery (FGUP "FCSSH Roszdrava") in Penza deployed Microsoft Office Communications Server 2007 R2 platform for remote online patient consultations and services. Case The previously accepted mode of interaction between medical staff of the Cardio Center and the N. N. Burdenko Penza Regional Clinical Hospital made the patient treatment process complicated and did not serve the needs of both clinics. There was a need for technology that would allow remote cooperation between physicians from different clinics. Solution The Microsoft Office Communications Server 2007 R2 solution allowed for the creation of a convenient, easy-to-use and low cost conferencing system providing remote online consultation capabilities to doctors at the Cardio Center and N. Lotsman is a Microsoft partner specializing in infrastructure and small business solutions. 1 In Penza, the Federal Cardio Center is the first in Russia to deploy the Microsoft Office Communications Server in order to provide permanent remote consultations within the region "Microsoft Office Communications Server 2007 R2 allowed us to achieve a new quality level of medical services. This solution allows for more efficient and accurate diagnostics and decreases presurgical patient death rate." Vladlen Vladlenovich Bazylev, Chief of Surgery, Federal Center for Cardiovascular Surgery The Penza Federal Center for Cardiovascular Surgery opened in 2008 as part of the national project "Health" and is one of the medical institutions providing high-tech medical services to cardiovascular patients in Russia. Some of the best Russian specialists from Moscow, St. Petersburg, Kazan and Krasnodar were involved in the work of the Cardio Center, as well as physicians from Penza, who received advanced training at leading Russian clinics in Moscow. About 2,800 surgeries are performed at the center for residents from 13 constituencies of the Russian Federation serving a population of 10 million people. The clinic is facing the challenge of using innovative technologies and methods of treatment. One of the steps to overcome this challenge was a regional initiative to provide the center with conferencing technologies for remote online patient consultation and services. This technology solution was implemented across the entire region. About the Project "This project provides doctors real-time capability to view films recorded by the medical devices, mark critical areas, and discuss the diagnosis and approach to treatment, thus lowering operational costs. In the past patients had to handdeliver their test results on recordable media from the Regional Hospital to our Cardio Center. Now there is no need for that." Vladlen Vladlenovich Bazylev, Chief Doctor, Federal Center for Cardiovascular Surgery The nature of the Cardio Center’s services requires remote cooperation with physicians at other medical institutions, specifically with specialists at the N. N. Burdenko Regional Clinical Hospital. However, the established structure of cooperation and speed of making joint decisions did not meet the needs of either clinic and undermined the efficiency of treatment. The purpose of the regional initiative was to provide the Cardio Center with technology for online consultations in order to improve the quality of medical services. Permanent telecommunication on the Microsoft Office Communications Server 2007 R2 platform was established between the two largest clinics in the Penza Region, the Penza Federal Center for Cardiovascular Surgery and the N. N. Burdenko Regional Clinical Hospital. Physicians of both clinics received a tool for remote cooperation with capabilities for real-time audio and video conferencing and data sharing. Case There was a number of challenges to overcome in the process of cooperation between specialists from the Cardio Center and the Regional Hospital. Among those challenges were: 1) Lack of coordination in making cooperative decisions on cardiovascular patients examined at the Regional Hospital 2) Increase in death rate at the presurgical stage due to the fact that patients had to hand-deliver cardiovascular test results (angiography) from the Regional Hospital to the Cardio Center 3) Lack of technology allowing remote decision making based on the angiography results Remote cooperation between the physicians of the two clinics using audio, video and web conferencing capabilities was the solution to these problems. Requirements of this undertaking included unified storage of data from patient tests, online communication between specialists (audio, video, and instant messaging), and tests result sharing. The goals were set based on the fact that specialists were unable to meet on a regular basis and losses in efficiency due to the fact that patients had to deliver test results on recordable media. The Microsoft Office Communications Server 2007 R2 platform was chosen as a foundation for the solution. The main factors for making this choice were the functional flexibility, simplicity of operation, high quality and low total cost. The audio and video conferencing hardware evaluation proved itself unsuitable due to either complex deployment or high initial costs. The analysis of Microsoft Office Communications Server 2007 R2 platform, on the other hand, proved that all requirements were met. 2 3 Solution "Development of high-tech medical service is a foundation for a high quality public health system. However, implementation of new technologies often requires large investment, which is so difficult to come during times of recession. One of Microsoft's priorities is to provide the Russian health care system inexpensive solutions that allow for the increase of quality and availability of medical services in Russian regions." It is a platform for instant messaging, user status notifications, conferencing and corporate telephone service. It provides live communication capabilities between colleagues regardless of their location. Employees get access to a simple and reliable set of tools for instant messaging, file transfer, voice and video. The user status next to the contact name in any Microsoft Office application shows whether a particular user is at his or her desk and the best way to get in touch with them. In addition, users can create a message about their status that can also help their colleagues choose the best method of communication and avoid wasting time by waiting or redialing. Engineers from Microsoft and a partner implemented the solution in two steps. company, Lotsman, The first step was the deployment of a pilot project in order to test the system in a real environment. The pilot project was deployed at the Lotsman premises. Specialists from the Cardio Center and the Regional Hospital were involved in the tests. Existing communication channels were tested for quality of voice and video stream with recommendations for optimization. The system was deployed on servers for a pilot run, hooked up to conferencing systems at both the Cardio Center and the Regional Hospital, and later adjusted to the needs of the organizations. The second step consisted of upgrading the Cardio Center's IT infrastructure, installing Microsoft server components required for running Microsoft Office Communications Server and Internet security measures (unified directory service Microsoft Active Directory® and secure and quick access server to corporate applications Microsoft ISA Server 2006). The system has capabilities to hold video conferences in rooms equipped with microphones, video cameras and a large screen for projecting images and patient test results from all participants. Every individual workspace is equipped with everything necessary to participate in the video conferences. Access connections to the Office Communications Server by means of the Microsoft ISA Server 2006 ensure confidentiality of communication via the Internet and security of data transfer. 4 Benefits "Microsoft Office Communications Server 2007 R2 allowed us to achieve new quality level of medical services. The administrations of both clinics as well as the regional office of the Health Ministry have already approved this technology solution. Simplicity and convenience in operation are the main benefits of this technology. Unlike similar industry specific solutions that require special equipment, Microsoft Office Communications Server 2007 R2 works on a regular computer and is less expensive. This technology solution currently covers two hospitals in the Penza Region but there are plans for future expansion to medical institutions of other regions.
