INTRODUCTION
The 8D (Eight Disciplines) Problem Solving Methodology is a step-by-step problem solving
methodology pioneered by Ford Motor Company through its Team Oriented Problem
Solving Global 8D Process. It is a highly effective approach to finding root causes,
development of proper actions to eliminate root causes, and implementation of permanent
corrective action.
In this 2-day practical workshop, participants will learn through a step by step
instructional process of preparation, establishing a team, brainstorming, to systematically
analysing and preventing problems from recurring. In addition, participants will learn the
practical problem solving tools required by the 8D process. Through the workshop,
participants will be able to appreciate why implementing and using an 8D problem solving
methodology using a multi-disciplined team is beneficial and important, especially when
meeting customer expectations.

THE FACILITATOR
Mr. Mahadev is a Quality Engineering Professional. After graduating with a Bachelor’s
Degree in Electrical Engineering from University Technology Malaysia in 1999, he has been
involved in various technical projects to optimize and improve manufacturing processes,
reduction in defects towards quality and productivity improvements at a Multinational
company in Penang in his capacity as an Assistant Manager. Mr. Mahadev is an American
Society of Quality Certified Quality Engineer (No: 45592), ASQ Certified Six Sigma Black
Belt (No: 3218) and TRIZ (Structured Innovation) Green Belt, Pretium LLC, US (No:SIT20037-10C). He is also a Certified Supplier Auditor (by SMMT) for ISO-TS16949:2002
Quality System. Currently, he is a Senior Member of American Society Of Quality (ASQ)
(No: 63458142), Senior Member of Society of Manufacturing Engineers (SME, US, No:
13889202), Member of Board of Engineers Malaysia (BEM, No: 59687A[Electronics]) and
also Member of Institution of Engineers Malaysia (IEM, No: G38860[Electronics]). Apart
from that, Mr. Mahadev has also been involved in “Lean Manufacturing and Six Sigma”
initiatives.
His expertise in manufacturing process variation reduction coupled with his leadership
skill and ability to integrate theoretical knowledge into practical real world situations
makes him a particularly effective resource person to impart knowledge in the areas of
Process and Quality Engineering.

REGISTRATION
Participation in the program is based on a first-come-first-served
basis. Cheques made in favor of the “FMM Institute” should be
forwarded on or before program date to the FMM Institute, Northern
Branch, Bandar Seberang Jaya. Participant who registered but did
not attend, will be billed accordingly. Upon confirmation, kindly
send us the payment before the commencement of the program.
CANCELLATION
There will be no refund for cancellation within 2 days prior to the
program, 50% for cancellation between 3 – 6 days and full refund for
cancellation 7 days prior to the program. Please inform in writing if
you intend to cancel. A replacement can be accepted at no
additional cost.
