Purpose of Position. The Inner South Communication Service supports the communication, participation and community inclusion of adults with disabilities and complex communication needs across five local government areas: Kingston, Bayside, Glen Eira, Stonnington and Port Phillip. This service is part of the statewide Communication Access Network, which consists of the Communication Resource Centre and 11 Regional Communication Services. The speech pathologist will provide clinical speech pathology services including working directly with individuals who have complex communication needs, providing peer support for other speech pathologists and delivering training. They will actively participate in the integrated health team s model of client centred service delivery providing direct consultation and consultancy to clients and colleagues. The speech pathologist will also be responsible for actively developing community awareness via community capacity building projects. 2. Key Responsibilities Clinical / Professional Build the capacity of generic services and communities to include people with complex communication needs. Deliver community-based, high quality speech pathology services to people with complex communication needs within the 5 LGA’s. Provide direct service delivery to clients with complex and chronic needs. Provide secondary consultancy to colleagues and peers regarding the area of expertise. Provide information and resources to clients, care-givers, other health professionals and the community. Provide peer support to speech pathologists working with people with complex communication needs Deliver training (formal and informal) to others who support people with complex communication needs. Advise other CBCHS services on issues relating to complex communication needs, augmentative communication and accessible information. Engage people with complex communication needs in all aspects of the program. Community Awareness Create sustainable communication environments and networks in services that support people with complex communication needs. Promote community awareness of Augmentative and Alternative Communication, diversity and inclusion. Develop and support regional networks to facilitate the inclusion of people with complex communication needs in the community. Develop and maintain partnerships with key agencies and individuals. Management / Team Responsibility Effective representation and participation in designated team meetings, case conferences/ reviews, client care, multidisciplinary and clinical meetings as requested. Contribute to the effective functioning of the service by ensuring that accurate and appropriate documentation of client information, statistics and educational material is maintained. This includes the maintaining of relevant databases including trakcare. Administer the service according to the policies outlined in the Service Specific Policy Manual Ensure that the service complies with the Victorian Disability State Plan 2002 -2012, the Disability Act 2007 and the Communication Access Network Strategic Plan Contribute to CBCHS planning and development and quality assurance activities as appropriate Actively participate and contribute to the ongoing development of effective working relationships with internal and external stakeholders. Active participation in the mentoring and development of students / graduates in the field of speech pathology as required. Identify, develop, implement and evaluate quality improvement activities in relation to team and organisational initiatives. Participate in projects, initiatives and other relevant duties, which are consistent with skill level, competence and training. 3. Health Promotion – Optional (except for Primary Health). Contribute to CBCHS health promotion activities and participate in developing relevant programs and projects in line with CBCHS Health Promotion Plan. Work collaboratively with other health professionals in developing a comprehensive multi-disciplinary health promotion approach to the local community as required. Community Participation Promote awareness to the community that CBCHS encourages consumer, carer, and community participation at all levels of the organisation. Participate in recruiting consumers, carers, and community in individual care, surveys, focus groups and committees as required. Work collaboratively with other health professionals in developing a community participation approach to service delivery. Key Selection Criteria A recognised qualification in speech pathology with eligibility for membership of Speech Pathology Australia Demonstrated professional experience as a speech pathologist working independently to achieve successful outcomes for clients. Demonstrated interest in working with people with complex communication needs Excellent written and verbal communication skills demonstrated by the ability to interact and negotiate with a wide range of people (families, support workers, community leaders, managers) A commitment to working in a collaborative way with others Lateral and creative thinking approach to problem solving A demonstrated ability and commitment to plan, deliver and evaluate services in line with the principles of the Victorian Disability State Plan 2002-2012 and the Disability Act 2007 Knowledge, skills and experience in project management A current driver licence Demonstrated skills and competencies in working with people with complex communication needs is highly desirable A knowledge of community capacity building frameworks is highly desirable 6.
