PROFESSSIONAL LEADERSHIP
Provide professional and clinical nurse leadership and expertise in development of Paediatric
Palliative care for children/young people with a broad range of life limiting degenerative
conditions requiring support from a specialist service.
Manage a specialist caseload of children/young people working across primary; secondary and
voluntary sector boundaries
Work with strategic partners and commissioning to establish, maintain and evaluate a
paediatric Palliative care pathway that meets organisational and strategic Island wide
objectives for Paediatric services
Ensures local standards of care for children and young people are benchmarked against best
practice national targets and/or international standards
.Work with Jersey Hospice Care as a key provider to develop pre bereavement services for
families of children/young people
Work in partnership with the Palliative Care MDT to develop a register of children/young people
following a Paediatric Palliative Care pathway
Promotes culture of change and innovation by influencing and managing change effectively
NURSING PRACTICE
Demonstrates understanding of, and upholds legal and ethical standards, NMC Code (2008).
Demonstrates in depth knowledge of distressing symptom management for children and
young people
Takes a significant role in raising the standards of care and quality of life for children and
young people facing all life limiting degenerative conditions expertise and excellence in
management of distressing symptoms across provider organisations.
Work in partnership with parents/carers of children and young people to provide multi
disciplinary assessment and holistic assessment and evaluation of individual needs, identifying
physical, social psychological and spiritual needs of children, young people and their families to
meet these needs
Work with key providers to ensure holistic assessment of needs at key points within the
Palliative care pathway that include the GP
Work in partnership with the named Paediatric Consultant as strategic lead and GP community
across secondary care to achieve seamless provision and continuity of service provision
Provide specialist care and advice for both inpatients and outpatients with long term or life
limiting conditions on individual care or treatment needs including pain assessment, and
provision of advice on the implementation of treatment plans from agreed formularies and /or
patient directives
Contribute to the formulation of evidence based documentation within policies and guidelines
to support best practice
Provide hospital discharge follow up to relevant patients with regard to their management plan
and liaise with a range of services as required
Manages a caseload incorporating continuous assessment to prioritise and be able to respond
to need for urgent crisis management where urgent isues occur
Provides accurate information, advice and support and signposting to children/young people;
families /carers and other practitioners as appropriate.
Acts as resource for parents and carers about the nature of the life liming and degenerative
conditions and support to develop effective coping strategies.
Works to achieve services that recognises the unique needs and voice of the child/young
person placing them at the centre of all assessment and decisions regarding their care, plan of
treatment and activities

Works collaboratively with other care providers and develop effective links and process for
referral across all provider organisations ie HSSD Paediatric Oncology Nurse, Paediatric Ward;
Jersey Hospice and others to facilitate service provision and or management of transition to
adult services.
Challenges practice that compromises the rights, privacy or dignity of children/young people
and their families.
Ensures all nursing documentation is comprehensive, logical, legible, clear and concise.
Utilises non medical prescribing knowledge and skills to support development and review of
agreed protocols, guidelines and Patient Group Directives for children and young people in
collaboration with Paediatric consultants and GP’s
Utilise specialist knowledge and skills to support the ongoing development of nurse
independent supplementary prescribing within the local context.
Act is the capacity as an independent supplementary prescriber to support pain management
and symptom control for children and young people with life limiting and degenerative
conditions
Keeps fully up to date and shares expertise with colleagues as required.
TEAM MANAGEMENT, BUILDING AND WORKING
Acts as a key member of the Palliative care MDT to ensure decision making and assessment
of specialist Palliative care needs for young people are agreed, documented and shared
across professional agencies as required.
Develops and maintains a collaborative working relationship with a range of provider agencies
across the Island where appropriate to support the care needs of children and young people
with life limiting and degenerative conditions.
Develops effective links and networks across the care provider environment by demonstrating
complex cross boundary working
Works across service areas providing Palliative care for children and young people both off
and on Island
Develops and maintains support with specialist teams and the child/young persons family
Empowers colleagues where required through facilitation, coaching and development providing
opportunity for effective professional discussion.
RESOURCE MANAGEMENT
Provide timely referral to Paediatric short break services and/or respite
Supports line manager in the development and review of key performance indicators for
service.
Produces high quality performance information and presents it at departmental/directorate
/commissioning level.
Is proactive in approach to own Performance Review and Appraisal (PR&A) and identifies
areas of own development need in agreement with line manager.
EDUCATION TRAINING and DEVELOPMENT
Provide education and awareness to families and young people on a pathway that are positive
and realistic
Provide an educational and practice focus in support of colleagues caring for children with life
limiting illness and any degenerative condition that experience distressing symptoms.
Leads initiatives to evaluate and meet the specific educational needs of both children, young
people and colleagues in relation to all aspects of care management of the patient group
Develop training and development plan for Paediatric pain management, review outcomes of
development plan and update in line with best practice to up skill key staff responsible for
palliative care delivery
Plan and organise support and education for other members of the wider community based
Paediatric services and workforce regarding control and management of distressing symptoms
Plan and organise specialist Paediatric Palliative care supervision
Works in partnership with a named UK service counterpart to obtain clinical supervision and
with Lead Nurse for Palliative care
Is working towards post graduate/masters award to support own role within the speciality
service.

Ensures own development includes currency of Level 3 Child safeguarding training and/or
updates
CLINICAL AUDIT AND RESEARCH
Work with key providers to adopts best practice standards to contribute to development of
integrated Paediatric palliative care pathway.
Works collaboratively with secondary; primary care and voluntary sector providers to achieve
best practice and quality of Paediatric symptom control and/or pain management
Applies a clinical governance framework to improving quality of care and principles of
safeguarding children and young people within an integrated Palliative care pathway
Assesses, identifies and intervenes proactively in circumstances that contribute to an unsafe
environment or practices for children, young people, families and /or staff. Bring instances to
the attention of line manager or named individuals regarding any safeguarding concerns.
Undertakes annual audit to establish objectives of own role and evidence achievement of
required outcomes for children, young people requiring the specialist service
Facilitate measurable and sustainable improvements in Paediatric palliative care services
through audit, projects, quality assurance and teaching initiatives.
Demonstrates critical analysis skills applied to best practice and translates same into
policies/procedures.
Monitors and reviews implementation of advice and treatment providing named expert contact
and advice for Paediatric colleagues in relation to specialist area of knowledge.
Provides annual report of service and activity for written and oral presentation at board level
Ensures currency of access and training for appropriate healthcare IT systems and are
accountable for “live” updating eg EMIS, Trak Care, path and radiology where appropriate, and
compliance with organisational policies.
Promotes a culture of openness, reporting and safety consciousness.
