All That’s Therapeutic is a family focused mental and behavioral health
agency dedicated to serving individuals, groups and families of all ages in

The mission of Baltimore Crisis Response, Inc. (BCRI) is to provide timely
and effective community-based psychiatric crisis intervention and
substance use treatment services in the most therapeutic and least
restrictive environment possible. BCRI serves Baltimore City with a

Building Families for Children is a faith-based, private, non-profit agency
located in Howard County, MD. CHOSEN is the treatment foster care
program of Building Families for Children, providing specialized treatment
homes for children, ages birth to twenty one years old, with special needs.
While headquartered in Columbia, MD we certify homes within a 50 mile
radius of the office, covering Montgomery, Prince Georges, Howard, Anne
Arundel, Baltimore and Carroll County. Children served present with
behavior problems, including but not limited to, impulsivity, depression,
aggression, runaway, truancy, lying, nightmares, stealing, enuresis,
attention deficit hyperactivity disorder, learning disabilities and certain
DSM IV psychiatric diagnoses. The children served have been determined
to be seriously emotionally disturbed, behaviorally disordered and/or
learning disabled. Treatment foster parents are trained in the use of specific
therapeutic techniques/interventions to increase pro-social behavior.
Employees in the CHOSEN program must be licensed Social Workers in
the State of Maryland. This agency offers on cite CEU training and
supervision for those looking to obtain their LCSW-C licensure.

The mission of Chase Brexton Health Care is to provide compassionate,
quality health care that honors diversity, inspires wellness, and improves
our communities. We are a Federally Qualified Health Center that provides
primary medical care, HIV testing and care, OBGYN services, addiction
treatment, mental health treatment, dental services, medication support, an
in-house pharmacy, and case management and outreach services.
Through Case Management and Outreach Department, our current Medical
Case Management program is available to all HIV-positive patients in need
of ongoing Medical Case Management services. Our case managers
complete an initial psychosocial intake assessment with all new HIVpositive patients, with an emphasis on medical needs, readiness, mental
health/substance abuse services, personal resources, and entitlement
eligibility. The majority of Chase Brexton’s Case Managers are licensed
social workers and 11 are also certified Assisters or Certified Application
Counselors (CACs), trained to enroll individuals in Maryland Medicaid
and Qualified Health Plans. In addition to monitoring patient insurance
eligibility, Medical Case Managers routinely refer their clients to other
services within Chase Brexton and to service providers throughout
Maryland. All referrals are monitored by the Medical Case Manager and
include frequent phone and face-to-face contact with the client and regular
follow-up with the provider or social service agency to ensure that the
client has been appropriately linked and that their needs are being met. The
goal of Medical Case Management is to help patients achieve positive
health outcomes, including viral suppression

The DC Child and Family Services Agency (CFSA) protects children from
abuse and neglect in the District of Columbia. Along with our community
partners, CFSA works to ensure that children involved in the public child
welfare system grow up in safe, permanent homes with strong families.
The Children's Guild serves children and youth in a number of programs in
the Baltimore/Washington, D.C. corridor - special education schools in
Baltimore and Prince George's Counties, therapeutic group homes for teens
in Baltimore City, treatment foster care for all ages in the greater Baltimore
area, and outpatient mental health services in Baltimore as well as school
based mental health services in 60 public schools in Anne Arundel,
Baltimore and Prince George's counties. Our work is based on
Transformation Education (TranZed) which is a strengths based
perspective focusing on the mindsets of the adults working with youth.
Excellent training and supervision are a key part of this approach. Our
organization continues to grow, thus offering opportunities for professional
growth and advancement.
The Children's Home (TCH) in Catonsville, Maryland, is a residential care
facility that provides both long and short-term services to youth 13 to 21
years of age who have experienced abuse, neglect, violence, abandonment,
and/or are in need of supervised care. TCH provides therapeutic programs
where children feel secure, loved and have a sense of belonging.
The Children’s Home programs and activities provide an opportunity for
the children to attend public schools, develop self-esteem, and experience
trusting relationships with peers and adults. Our programs focus on
enhancing each child's ability to function successfully in society with
attention to accountability and community safety.

Choices provides management, support and training for communities with
the goal of keeping youth in their homes and communities. A national
leader in systems of care development, Choices is at the forefront of a
nationwide effort to serve youth involved with the child welfare, juvenile
justice, mental health and special education systems. Hundreds of youth
and families are served annually through our broad spectrum of programs.
Community Connections is the largest not-for-profit mental health agency
serving men, women and children in the nation's capital. Since its founding
in 1984, Community Connections has been committed to innovative and
compassionate mental health, addictions and residential services for the
District of Columbia's most vulnerable citizens. Consumers also present
with other concerns which may be compounded by mental health
symptoms. These include co-occurring problems of addiction,
homelessness, medical illness and violent victimization. Integrated clinical
services include diagnostic assessment, community support, care
coordination, therapy, and liaison with psychiatry clinic services.
Community Support Specialists work with consumers to address symptoms
and deficits that prevent recovery from mental illness and interfere with
other goals. Specialty teams that are a part of the adult service program
include: Trauma, Dual Diagnosis, Justice, and Recovery Community
Teams
The Community Connections ACT teams serve people who suffer from the
highest intensity of mental illness and co-occurring problems; consumers
who have had frequent medical and psychiatric hospitalizations, arrests,
and episodes of homelessness. The ACT Program embraces the nationally
established Evidence Based Practice Model for this level of care.
The Community Connections Child and Adolescent Services Program is
youth-centered, family focused, and community based. Our goal is to
provide high quality and culturally competent mental health services that
are focused on the strengths of each individual child and family.
Community Support Specialists meet the child where he/she is, learn the
values and perspectives of the family, and join with them in discovering
and affirming skills, abilities and talents.
The CBI program at Community Connections provides time-limited,
intensive services to help improve the lives of children with complex needs
and reduce the likelihood of more restrictive settings including and
especially out-of-home placements. The CBI model targets service to
specific children but works with the entire family.

For more than seven decades, Lutheran Immigration and Refugee Service
has been a champion for migrants and refugees from around the globe. Our
legacy of courageous and compassionate service has made a difference in
the lives of hundreds of thousands of people who have sought safety and
hope in America’s communities. Our history reflects American Lutherans’
deep immigrant roots and passionate commitment to welcoming
newcomers.
LIRS works with migrants and refugees, U.S. Lutherans, and many
partners in service and justice to accomplish our mission. Refugees are
resettled through a network of 28 local organizations in 26 states, most of
which are Lutheran social ministry organizations. Sixteen of those
organizations also provide immigration legal services. Asylum seekers,
survivors of torture and other migrants impacted by detention are served
through 23 partners providing legal and social services including visitation
ministry. In addition, we work directly with eight foster care programs at
the state level to provide family reunification and foster care services for
unaccompanied refugee and immigrant minors.

Clinical and The Department of Juvenile Services (DJS) is an executive agency whose

primary task is to appropriately manage, supervise and treat youth who are
involved in the juvenile justice system in Maryland. DJS is involved in
nearly every stage of the juvenile justice process from the moment a youth
is brought into a juvenile intake center by the police or as a result of a
citizen complaint to the time when a youth returns to the community after
completing treatment.
By law, DJS is a child-serving agency responsible for assessing the
individual needs of referred youth and providing intake, detention,

Park West Health System, (PWHS) is a designated Federally Qualified
Health Center that operates four urban health centers that provide
comprehensive health care to the medically underserved residents in

and mental health treatment, HIV disease, Hepatitis C and specialty care

referrals. PWHS’ longstanding commitment to primary care has always

in line with HRSA priorities, including family-centered care, team
approach that integrates medical and social work professionals, and
incorporating cultural competency and the ethical values imbedded in
serving the underserved. PWHS understand that, for both individuals and
families, health depends not only on health care but also on other factors
including individual behavior, genetic makeup, exposure to health threats,
and social and economic conditions. In order to encourage more lowincome and uninsured residents in our service area to seek appropriate and
cost-effective primary and preventive health care from one of our four
sites, PWHS’ staff conducts regular outreach and preventive health

Pathways to Housing DC is a housing-first model program that helps
individuals with severe mental health and other co-occurring disabilities
with maintaining their wellness recovery. The program helps individuals
get connected to subsidized housing vouchers and then provides intensive
community-based case management services. Pathways to Housing
provides clinical interventions using the Assertive Community Treatment
(ACT) model by have a multi-disciplinary team that includes medical
(nurse, Peer Health specialist), addiction (CAC) and Social Work staff to
provide support to our consumers.
Powell Recovery Center provides integrated addictions, medical and
mental health treatment to public health clients in Baltimore City. Powell

Woodbourne Center provides psychiatric residential treatment for boys,
ages 12-18, who have serious mental health problems and have exhibited
harmful sexual behaviors. Specialized programming includes two sexual
trauma and recovery units and other comprehensive treatment units.
The treatment approach is multidisciplinary and collaborative, utilizing
cognitive behavioral therapy, individual, family, group, and milieu therapy.
All programming is trauma-informed and family-centered.
