To ensure holistic care is provided, this position will apply contemporary nursing practices, liaise with care staff and provide supervision to Levels 1 and 2 Registered Nurses. The position holder is responsible to ensure that services provided are consistent with the mission and values of Churches of Christ in Queensland. Facilitate participation of the Centre for Excellence in Dementia Care in external research projects as approved by the General Manager Residential Aged Care. Participate in all Centre for Excellence in Dementia Care activities as required. Provide clinical expertise to stakeholders and staff of the Centre for Excellence in Dementia Care project. Operationalise the Cottage Life Model of Care in alignment with the vision of the Centre for Excellence in Dementia Care by: - ensuring a prosthetic environment - maintaining resident function - implementing person focused care Continue with evaluation activities beyond the life of the project. WMS-01 Manage care-related portfolios (eg. Continence, infection control) as delegated by the Service Manager. Coordinate assessment, planning, delivery, communication and evaluation of resident care in accordance with Churches of Christ Care’s care management systems and processes. Coordinate, delegate and monitor resident admission processes, including involvement in effective resident admission to direct care staff. Coordinate, delegate and monitor assessment and recording of resident’s care needs, including history taking, baseline assessment, focal assessment, physical assessment, determining care needs and referral to other members of the allied health care team where indicated. Coordinate, delegate and monitor the development of individual resident care plans, including determining care needs, establishing care goals in consultation with direct care staff and other members of the allied health care team. Coordinate the delivery of Registered Nurse specific clinical care in accordance with each resident’s plan of care and coordinate the activity of direct care staff in delivering care and/or Level 1 and 2 Registered Nurses. Coordinate, delegate and monitor regular, comprehensive, outcome-based evaluations of resident care, in consultation with the direct care staff and other members of the allied health care team. Coordinate referral to allied health providers as indicated in accordance with the Service policies and procedures. Coordinate, delegate and monitor consultation and communication with residents, their relatives and significant others in assessment, planning, delivery and evaluation of care and referral to allied health care providers. Coordinate and promote effective infection control by with direct care staff and Level 1 and 2 RNs. Participate in the coordination of recruitment, induction and rostering activities for care staff, in consultation with the Service Manager. Coordinate annual reviews and ongoing performance assessment and management, for all care staff in consultation with the Service Manager. Attend to staff replacements as delegated by the Service Manager. Facilitate the professional development of direct reports, in consultation with the Service Manager. Other activities as required by Service Manager Key Selection Criteria KSC 1 Possess registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse with a current Practicing Certificate. KSC 2 Possess a current National Police Certificate (no more than 3 years from date of issue). KSC 3 Possess a current Senior First Aid Certificate with resuscitation techniques. KSC 4 A post graduate qualification in dementia specific care is desirable KSC 5 Demonstrated experience in the aged care sector, for a minimum period of 3 years, including care of residents living with dementia. KSC 6 Demonstrated ability to effectively implement significant change at an organisational level, and to develop a work team though the change process. KSC 7 Demonstrated ability to identify customer requirements, alter services in response to identified needs, and evaluate customer satisfaction. KSC 8 Highly developed problem solving, decision making and negotiation skills. KSC 9 Applied knowledge of ACFI and Accreditation requirements.
