JOB DESCRIPTION Job Title: Nurse Practitioner/Advanced Practice Nurse Reports To: FLSA Classification: Physician Designee, Administrator and Clinical Director Exempt BRIEF POSITION SUMMARY: Provides health care to patients in the hospital or office setting. Works in consultation, cooperation and coordination through a collaborative physician. Functions according to the established standards and practices of medicine for Advanced Practice Nurse/Nurse Practitioner in the State of [state]. Identifies the need for additional data collection including the need for data collection from other health providers; Documents findings and actions appropriately in the patient's medical records. Assists in educating the patient and family members regarding medical condition, procedures, and medication and preventative measures as necessary and appropriate. Recommends community resources to meet patient and family needs. Participates in various MMG initiatives such as clinical research, CHF clinics, weekend coverage .etc. Triage patient’s telephone calls and provides consultations and/or recommendations for appropriate treatment. Performs evaluations and treatment procedures in emergency situations when necessary. Maintains patient confidentiality. Attends required meetings and. participates on committees as requested. Participates in professional development activities and maintains professional affiliations and CME's Performs related work as required or assigned. Degree in Nursing and Primary Health Care or Cardiology is required. Experience: A minimum of 1 to 3 years work experience as a Nurse Advancing Leaders. Certification: Current certifications in BLS, CPR and ACLS are required with current RN license and Nurse Practitioner licensure with current certification from the American Association of Nurse Practitioners, (AANP) or the American Nursing Credentialing Center, (ANCC). 3. Special Skills: Current BLS and ACLS certifications are required. Above average skills in clinical procedures are required. Understanding and ability to follow OSHA requirements and provide instruction to staff on safety hazards and precautions are necessary. Must understand general insurance protocols and applicable billing guidelines and regulations. Professional verbal and written communication skills are required to establish and maintain effective working relationships with patients, physicians, team: members and the public. PHYSICAL DEMANDS: Lifting: Average weight 10-15 pounds for a distance of up to 4 feet approximately 110 times per hour. Occasionally up to 250 pounds as needed for patient transfer with assistance of other staff to prevent potential injury to employee. Carrying: Average weight 1-25 pounds for a distance up to 75 feet approximately 1-15 times per hour. Standing in place: Approximately l hour per shift for a duration of 1-2 hours at a time on carpeted or tile surface. Sitting: Approximately 1-2 hours per shift for i.t duration of 1-2 hours at a time. Walking: Approximately 1-2 hours per shift for a distance of 10 to 75 feet at a time on carpet or tile surface. Reaching: Overhead to a height of approximately 10 feet as needed per hour. Bending: Waist to floor 15-25 times per shift.
