INSTRUCTIONS: Please use this form to submit your RAMP proposal. Fill-in complete answers for all
questions. Submit complete proposals along with your completed RAMP budget form (see below) and PA
Industry Partner support letter on company letterhead no later than September 10, 2012.

** Note that “support letters” are required to be attached to this proposal from all participating PA
companies. Participating companies that do not provide a support letter will not be counted as
participatory during the proposal selection process. The support letter should address the impact
of the RAMP project on their operation and the significance of the technology/process being
proposed and should outline the financial contribution (cash or in-kind and respective quantity
and description of each applicable type of financial contribution being proposed). Note that
sections from support letters may be included in RAMP Program reports provided to the
program’s funding agency in order to emphasize the impact of the program on PA manufacturing
companies.
