HEALTH REQUIREMENTS FOR CHILD DEVELOPMENT CENTER REGISTRATION
We welcome your student to the Los Alamitos Unified School District-Child Development Center and hope he/she will have a very healthy and happy year! To ensure the optimum health of students and to comply with district, county, and state requirements, please note the following:
1.
TUBERCULOSIS TEST FOR SCHOOL
All students entering the CDC program must be screened for Tuberculosis by a physician.
2.
IMMUNIZATIONS FOR SCHOOL
According to state law and school district policy, no student will be allowed to enroll into any district school without proper evidence of full immunizations. Immunization requirements are as follows:
1. Polio: a minimum of 3 doses received at the appropriate intervals:
2nd dose: 6-10 weeks after 1st dose
3rd dose: 6-12 months after 2nd dose
4th dose: Required only for entry to Kindergarten level or above. Age 4-6: if 3 rd
dose
was given before 4 th
birthday, one more dose is required before admission. Students 7
years and older-3 doses meet the requirement if the last one was given after 2 nd
birthday.
2. DPT: (or combination DPT, DT, or td):
A. Students under age 7 years — a minimum of four doses at the appropriate intervals):
2nd dose: 4-8 weeks after 1st dose
3rd dose: 4-8 weeks after 2nd dose
4th dose: 6-12 months after 3rd dose
5th dose: needed only if 4th dose was given before 4th birthday
3. Measles, Mumps, Rubella (MMR 3 )
One dose, on or after 1st birthday
4. Hepatitis B:
Three doses

5. Varicella:
One dose
6. HIB:
One dose
Students who have not yet completed the immunization series are allowed to attend school only on condition that they are in progress and up-to-date in the sequence of required immunizations.
Exemptions from immunizations are given with a signed statement that they are contrary to the parent’s beliefs or with a signed physician medical exemption.

SCHOOL PHYSICAL EXAM FOR CHILD DEVELOPMENT CENTER
All enrolling Child Development Center students are required to have a physician fill out a
Physician’s Report-Child Care Centers (Child’s Pre-Admission Health Evaluation). The report must be done within 30 days of enrollment and no earlier than 1 year prior to enrollment.
