Ortho needs assessment and procurement update NHS England Cherie Young
• CY updated the LDC on the current situation all PDS orthodontic contracts (72 in total) come to end on 31st
march 2016.
• In Kent, Surrey and Sussex the LAT have completed a mapping exercise looking at where procurement is
needed.
• Although the orthodontic needs assessment is complete wider consultation is required before procurement, Local
Office of NHS England South(South East) Region has applied for a 2 year extension to the contracts.
• This ‘interim’ period is an extension of the existing contracts but there will be contract variation with bolt on
clauses. Orthodontic providers will keep their UOAs but their contracts will have clauses in the such a maximum of
22 units per COT (Course of Treatment), and UOAs will not be transferable. LOS(SER) trying to prepare
orthodontic providers for what the future will hold.
• The orthodontic procurement needs to be done correctly, accurately and appropriately and be the last one.

Referral Guidelines - MCN tomorrow will present a referral pack; one for the orthodontists and one for GDPs to
refer to orthodontists.
This will be for Kent, Surrey and Sussex. One uniform referral form.
Soon this will be electronic via Vantage. Kent initially.
Patients will ONLY be entitled to one COT of orthodontic treatment. If they refuse/ decline this, then they will not
be able to access NHS orthodontic treatment again although an appeal process will exist.
GDPs will need to show patients a list of the practices and their associated waiting times in order for the patient/
patient’s family to make an informed decision of which orthodontist to be referred to.
GDPs will need to take an IOTN assessment but there will be a system to help them do it.
HEKSS will provide courses (free) in each area for GDPs to train with the taking of IOTN.
This referral system will come in as soon as MCNs sign it off.
Last year £1.8million was spent in KSS for not fitting an orthodontic appliances which is a significant impact on the
public purse.
When the orthodontic tender comes out, the bid needs to be a realistic bid to win. The UOA is not getting reduced.
Providers are asked to put an accurate bid in knowing what a transfer case costs, etc.
• The Implementation group is a national group there to iron out any issues.
• Samit Shah (SS) expressed concerns that age is currently a barrier to orthodontic treatment since adult
orthodontics on the NHS is not permitted.
• CY stated that our orthodontic colleagues are currently under extreme pressure and will return orthodontic
referrals if inappropriate. Some orthodontists may offer ‘lunch and learns’ to educate GDPs
• Sedation Update
• Within a month, GDPs will receive an email stating the sedation clinics and their waiting list times.
• Concerns were discussed about the use of poly-pharmacy , list will also state which practices are operating polypharmacy.
• An anxiety score assessment will be needed for patients being referred for sedation.
• St Faiths in East Grinstead are closed to accepting referrals (except children under 16) from West Sussex for
XLAs/ filings under sedation, so all referral will be diverted to Steyning (Southern Dental). St Faiths will still accept
IMOS XLAs under sedation. 60% of referrals from Kent and Surrey.
• New RCS guidelines were mentioned, it is unclear yet whether NHS England or local sedation practices will be
adopting them
Health Education Kent Surrey Sussex Dr Sam Shar
• An update on older people’s project: it is in the pilot stages currently.
• Pilot phase; aims to develop resources, deliver training, work alongside SALT. The focus is prevention.
• An evaluation is being carried out to monitor its effectiveness.
• The Health Education Team deliver training.
• Foundation dentists to go to care home to get a flavor of how treatment works in this setting.
• If the project is successful, they will advise the CQC on inspections concerning dental aspects to care homes.
• Mouth-care matters is a program to support OH in inpatient setting, nurses can achieve a diploma
• Wider engagement with current providers for sustainability is happening more now.
• SS mentioned we should meet with the electronic referral system developers (Vantage) for us as an LDC to give
them our ideas concerning the new system. This was welcomed.
Special care dentistry: SCT Rob Emmanuel/ Parul Patel
 CQC visit : SCT got a good
 We are currently recruiting for an SDO in JDC, Chichester.
 We have recently recruited two dental therapists, one of which is on a fixed term 1 year contract
 We are currently undertaking a few research projects/service evaluations with Brighton Medical School,
Chailey Heritage and University of St. Andrews
 We are currently undertaking the stakeholder engagement for the relocation and integration of Conway
Court Dental Clinic in Hove to Morley St. Brighton.
 The emergency dental service (EDS) is continuing to audit the dental practices who have a
disproportionately high level of registered patients attending the EDS due to not being able to acquire
appointments within the working week.
 Oral Health Education is being updated and in line with National Diploma examination structure for theory
side of exam. The OSCE scenarios will also need to be updated.
 Rob and Sarah are members of the recently established Managed Clinical Network for the region and will
be working on developing and shaping SCD in the future.
 recently published Standards for Conscious Sedation and the Provision of Dental Care 2015 Report of the
Intercollegiate Advisory Committee for Sedation in Dentistry i.e. published on 22nd April 2015 this report
creates a new national standard for conscious sedation in dentistry. Focused on patient safety and optimal
patient care recommendations are made about information for patients, robust validated education and
training for the entire dental team, sedation techniques and the appropriate environment for sedation
delivery.
There are some significant changes to previous guidance. The report defines the age of a child and the
requirements of the clinical team and facilities required for the dental treatment of younger patients under
conscious sedation.

Just to remind that the paediatric dentistry service is only funded for 6 outpatient sessions monthly.

MFU/ MOS Up-date: by e-mail rom Steven Walsh
The primary care MOS cases have started to come into SRH. We have had some early teething problems but now
all is running a bit better. This is a good thing having taken so long.
We still have yet to recruit for Mr Macphersons post and will re-advertise soon so we are still effectively 1.5 WTEs
down at consultant level.
Unsure of how the new Triage system will yet impact on us
Restorative Up date :by e-mail from Dr AJ Ray-Chaudhuri
A quick update for Restorative in BSUH, AJ works still part-time and single handed (Wed-Fri).
Accepting patients with HANC, Developmental disorders (e.g. CLP) and orofacial injuries following trauma for
complex care regardless of geographic boundaries. Most are referred from Consultant colleagues e.g. Brighton,
Eastborne, Worthing etc. Simple care is returned to the GDP.
Referrals are accepted from Brighton and Hove PCT only, for normal Restorative cases e.g. Perio, Pros, Implants.
Almost all patients are returned with a treatment plan to the GDP. 7-14 patient slots available per month.
Endo is sent to the DwSi and triaged by the Dental Advisor. In the future would be keen to engage with clinicians
who may wish to become clinical assistants 2- 4 sessions per month. This would be unpaid in the first instance but
with excellent educational support.
