The service aims to provide the most appropriate intervention for each individual patient. Some of the solutions that the Medicines Concordance Team can provide are: - Clarification of medication routine - Suggestions for medication prompts e.g placing by bed or kettle, alarm clock, how to obtain specialist alarmed dispensing devices - - - Labelling modifications e.g large font size or additional labelling. Container modifications e.g blisters transferred to bottles, plain caps Referral to Community Pharmacy for a Medication Use Review (MUR) What should you do if you or a relative needs some help with medications? Ask a healthcare or social care professional to refer to us. What referral information needs to be provided? The aim of the Medicines Concordance Team is to ensure that patients are able to take medication safely and effectively in their own homes. We need as much background information as possible which helps us to manage each referral appropriately and effectively in a timely manner. The following information is vital and is requested on our referral form: Who provides the Service? The service is run by a team of Registered Pharmacy Technicians with experience in this field. - Patient details - GP details - Pharmacy where medication is dispensed. - Specific details about what problem the patient has with their medicine. What type of patient can be referred? Any patient over 18 who is registered with a GP in the Thanet area and who is finding it difficult to take their medication correctly can be referred. - Reducing hospital admissions - Reducing harm from medication - Improving the safe and efficient management of medication across the primary / secondary care interface - Reducing prescribing costs due to waste What can the service provide? The Medicines Concordance Team will assess each patient that is referred. This may involve: - Liaising with other agencies e.g. GP, pharmacies, Social services, Care Agencies - Involving family or friends where appropriate Provide support to patients with long term conditions, e.g. dementia and mental health conditions Undertaking compliance and concordance review using GP information initially and then involving the patient - - Clarifying medication discrepancies on discharge from hospital. The provisions of services to enable older people to live independently - - Partnership working with social services, social care and community healthcare providers Usually visiting the patient at home to conduct an assessment to establish how the patient is managing. - Details of anyone that may help the patient with their medication. - Any hospital admissions What will the service do? Who can refer patients? - Any Healthcare Professional. How can referrals be made? A Medicines Concordance Team referral form will need to be completed in full.
