By signing this document __________________________________________ acknowledges,

agrees to and is in full understanding of the driver ride-along policies set forth in this document.
This is to be valid for any ride-along passenger(s) for any length of time at the driving employee
of Bowers Trucking, Inc. discretion.
Driver ride-along, passenger, family member, spouse, friend, associate, etc. is strongly urged
against in day-to-day operations within Bowers Trucking, Inc. However, such a corporation has
sympathized with the circumstances of its driving employees and has allowed, only on occasion,
a ride-along passenger to attend and witness the duties carried out by a Bowers Trucking, Inc.
employee throughout the duration of the working period. By signing this document the above
mentioned Bowers Trucking, Inc. employee waives all liability of fault, damages, physical harm,
dismemberment, death and/or any result of any accident or incident etc. It is the sole liability
from this point forward of the employee/ driver of Bowers Trucking, Inc. to provide medical
physical and/or mental care of the ride-along they choose to have present with them at any time.
There is to be no liability, fault or negligence directed toward Bowers Trucking, Inc. in the event
of an accident, incident, or any occurrence that may cause mental, physical or medical harm to
the ride-along in the presence of the Bowers Trucking, Inc. facility, tractor/ trailer, equipment,
job site, customer plant site, pick-up site or delivery site. Furthermore, it is the sole and sworn
duty and liability of the Bowers Trucking, Inc. employee to provide such medical, physical or
mental care to the ride-along of his/her discretion in the event of any type of misfortune for the
ride-along passenger. This document waives all liability, fault and negligence that may or may
possibly be brought forth in any type of litigation that could or might be pending against Bowers
Trucking, Inc. for the benefit or care of the ride-along passenger and/or his or her family, friends,
associates, guardian or estate.
The signatures below reflect the submission of all parties involved and signifies the agreement
and understanding of the provisions set forth in this document without any coercion, duress,
force or compensation.
