Important: Applications must be typed. Handwritten applications will not be accepted.
Prior year forms will not be accepted.
1. This scholarship is available to SRI 4-H members only who have participated in the SRI 4-H Fair.
Members must be accepted to, or attending, a two or four year accredited higher education institution
for an undergraduate degree (minimum 12 credits or equivalent full time course load). Members
may receive the SRI 4-H Fair Scholarship no more than two times, or two years.
2. The SRI 4-H Fair Committee has allocated four $500 scholarships in 2015. The SRI scholarship
committee will determine the scholarships recipients. Scholarship recipients will receive half of the
scholarship amount awarded at the fair. The second half of the scholarship award will be mailed to
scholarship winners after a copy of the first semester’s passing grades (GPA of ‘C’ or better) have
been provided to the scholarship committee chairman.
3. Transcript of latest grades and a personal letter of recommendation from your club leader or 4-H
mentor are both required with this application.
4. DEADLINE DATE: Application must be postmarked by June 1, 2015.
5. In fairness to everyone, late applications will NOT be accepted.
6. Candidate interviews will be held by the scholarship committee on Saturday, June 20, 2015 at 9AM
in the West Greenwich Town Hall Council Chambers located at 280 Victory Hwy, West Greenwich.
It is highly recommended that interested scholarship candidates attend the interview. Candidates
who are unable to attend the interview at this time will be considered based solely on the merits of
their application.
7. Scholarships will be announced at the fair on Sunday afternoon after Premier Showmanship.
