Expression of Interest Integrated Elderly Care Public-Private Partnership Pilot Project In Yichang City, Hubei Province Submission deadline: 8:00 am, January 15, 2016 (CST) Cities Development Initiative for Asia (CDIA) is a regional initiative established in 2007 by the Asian Development Bank and the Government of Germany, with additional core funding support of the governments of Sweden, Austria, Spain, the Shanghai Municipal Government and Switzerland. The Initiative provides assistance to medium-sized Asian cities to bridge the gap between their development plans and the implementation of their infrastructure investments. With the funding support from Shanghai Municipal Government, CDIA calls for expression of interest of qualified companies to carry out the technical assistance for the aforementioned PFS work. Background of the project There are strong needs of Elderly Care in Yichang city. Yichang has 4.15 million inhabitants, ou of which 20.04% were over 60 years of age in 2014. The elderly ratio is already above the national average of 13.7%, and in urban Yichang is projected to rise to 26% by 2030. Surveys have found that around one-fifth of Yichang’s elderly need assistance in their daily living, with 4% of the elderly severely impaired. Slightly more than half of the elderly live by themselves or with their spouse without any other family member taking care of them. In those ‘empty nests’, seniors are exposed to insufficient care for daily life, lacking help for house chores, making hospital visits, etc, as well as a lack of emotional support. There is a mismatch of needs and services. Yichang’s 170,000 urban elderly are supported by 11 public residential care facilities with 1,505 beds and 19 private residential care facilities with 2,181 beds. Occupancy rates are low—57% in public facilities and 37% in private facilities—which is attributable to high fees in some facilities, and low quality in others. At the same time, around 60% of the residents in urban residential care do not actually need long term care services, with many remaining there because of inadequate home-based support. The result is a mismatch of needs and services. A considerable number of elderly with residential care needs are living in communities while many in residential facilities do not need to be there. Services need to be expanded and improved. Yichang requires a substantial expansion in the elderly care (EC) services and better system design. There is a need to improve the effectiveness of residential care and develop community and home based alternatives that provide the elderly an attractive and affordable integrated system of support. This needs to provide continuous care in accordance to the level of functional disability. A much better interface is required with the medical support offered by geriatric and acute care hospitals. Objectives of the Consultancy Services With the help from CDIA, Yichang municipal government is aiming to build EC services that is affordable to both users and YMG needs. The outline concepts have been prepared by ADB and discussed with YMG for a geriatric hospital, residential-based community care facility, and elderly independent living. These three concepts demonstrate how the PPP approach can be applied in institutional, community, and home-based care. The goal of this consultancy assignment is helping YMG to design an Integrated Elderly Care system which is affordable to both users and YMG needs, structuring the project under PPP modality which could become an Elderly Care Pilot PPP Project. It will also help to identify potential private partners for cooperation, and within this scope: Project structure. Study on how to design, build, operate, manage and maintain the EC services and facilities. Structure different parts of the project in packages that create economic and service delivery sense for further bidding. Design a suitable schedule on project implementation. Broaden financial resources and external expertise. Customize PPP scheme to attract private enterprises, which can build sound and effective EC services and facilities, whilst improving the EC standards in Yichang based on sustainable operation and long-term development. To explore suitable patterns of agreements for EC services. Identify project stakeholders and potential partners. To analyse what services can be provided by private partners. Provide recommendations on payment scheme. . To analyse project investment proposal, revenue estimation and viability, and improve the project financial proposal. Identify and assess key project risks, analyze how to prevent or mitigate risks. To analyse project environmental and social impact, particularly the social impact and its benefit for marginalized group and poor people. Provide capacity building training for relevant stakeholders on PPPs. Introduce best practices on EC PPP, including approaches on business running, model of management etc. More details of the scope of PFS will be described in the Terms of Reference that will be provided to shortlisted consultancy firms in the Request for Proposal.
