BSN Student Health Passport Requirements

Summer 2015 Deadline Dates: Place your order for the Background Check + Drug Screen + Medical Document Manager no earlier than 4/1/15 and no later than 5/15/15 and upload all documents by 5/15/15.

**Failure to meet these deadline dates will result in forfeiture of your admission for the Summer 2015 term.

For students to be eligible for participation in laboratory and clinical rotations throughout the program, each individual is responsible for satisfying and maintaining compliance to the Health Passport Requirements prior to the first day of each semester. Please be aware that non-compliance of Health Passport Requirements will exclude students from the Integrated Experiential Learning (IEL) course until the passport is compliant.

Each student is required to manage a Certified Profile Account (Background Check + Drug Screen + Medical Document Manager). To enroll, place an order on www.certifiedbackground.com

using package code: if78. To maintain IEL lab and clinical eligibility, each student is expected and responsible to ensure that all requirements are “Compliant” and will not lapse during the semester as

“Overdue” or “Rejected.”

Upload the below requirements to your Certified Profile Account with official documentation. Lab Reports, Clinic Record, Employee

Record, School Record, State Alert System Report, or Immunization Card (with date and provider’s signature, initials, or stamp included on the card); or Provider letter is acceptable.

Measles (Rubeola), Mumps, and Rubella (MMR): Complete either “a” OR “b” below: a.

2 MMR Immunizations, at least one month apart. Students may attend class during one-month waiting period between immunizations.

OR b.

Positive titer (blood test) for each: Dates of positive titers for Measles (Rubeola), and for Mumps, and for Rubella.

Varicella (Chicken Pox): Complete either “a” OR “b” below: a.

2 Varicella Immunizations at least one month apart. Students may attend class during one-month waiting period between immunizations.

OR b.

Positive titer (blood test): Date of positive titer.

6.

Hepatitis B: Complete either “a” OR “b” below: a.

Positive titer (blood test) after receiving 3 immunizations:

***Students may start the series now and continue in class as long as each step is completed on time, including the final titer to determine immunity. i.

Date of First Immunization; ii.

Date of Second, 1 month after first shot; iii.

Date of Third, 5 months after second shot; iv.

Date of Positive Titer, 6 weeks after third shot

OR b.

Sign Hepatitis B Waiver : signing a waiver may limit clinical site options

Tetanus, Diptheria, and Pertussis (TDAP): within past 9 years (booster required every 10 years); a DT is not accepted; the

Pertussis must be included.

8.

Tuberculosis (TB) Screening, including Latent TB. Must be valid entire semester. Must be screened for recent exposure annually. A previous positive Skin Test or IGRA Test must be followed up with a Chest X-Ray.

NOTE: complete your TB tests before getting any immunizations as you will need to wait 4-6 weeks after an immunization to complete a TB skin test.

Complete either “a” OR “b” OR “c” a.

2-Step TB Test : 2 distinct TB Skin Tests; the second TB skin test must be no earlier than 1 week after the first test was planted and no later than 3 weeks after the first test was planted. (If you have documentation of an earlier 2-Step

Test or IGRA, you will only need an annual 1-Step TB Test). i.

Upload the document for the first skin test date and results and the document for the second skin test date and results together.

OR b.

IGRA Test (blood test): Quantiferon Gold or T-SPOT TB test specifying date and negative results; if positive, a chest x-ray is required.

OR c.

Chest X-Ray: upload the document of a clear chest x-ray that was taken within 12 months prior to starting the nursing program. Complete an Annual TB Review of Symptoms Form every year thereafter. Repeat chest x-rays are not required as long as there are no active symptoms.

Annual Seasonal Influenza Vaccination: a.

Provide official documentation OR complete the Personal Certification form .

10.

CPR: must be valid entire semester: a.

BLS for Healthcare Providers Card: Basic Life Support (BLS) for Healthcare Providers through the American Heart

Association is the only certification accepted. Online “Cognitive” skills must be accompanied by a hands-on skill test; online certification without the hands-on skill test is not accepted.

11.

Annual StudentMax Orientation Quiz a.

Complete the StudentMax Orientation Quiz and Upload the Certificate i.

Select “Take The Quiz” vi.

Set up a Quiz Account vii.

Select “Quiz Catalog” viii.

Enroll in “1-FERPA Release” and “2-NSGMDPA General Orientation Quiz AY2014-2015” ix.

Print, scan, and Save the Quiz Certificate as a PDF x.

Upload the Quiz Certificate to your Certified Profile Account
