WHAT DO I NEED TO DO?
Fall 2013
1) Find out what kind of insurance you havea. Get a copy of your card if possible
b. Get the date of birth for the person who carries the insurance
c. Get the name of the employer for the person who carries the insurance
d. If you cannot get the card right away, you will need:
i. Your policy number
ii. The group number
iii. The insurance company phone number
2) Investigate which therapy service you are interested in attending.
3) Contact therapy office to schedule your first appointment
a. Ask if they accept your insurance
b. Ask if there is paperwork you need to complete prior to the first appointment
c. Ask when your co-pay is due
d. Ask options for paying your co-pay
i. Cash/credit card/check/debit cards
4) Call your insurance company (the number is on the back of your insurance card)
a. VERIFY the therapy center/therapist is an approved provider.
b. Ask: Do I have a deductible- this is the amount you have to pay prior to the insurance company
paying?- How Much?
*Aetna ISU student insurance has a $450 deductible.* YOU MUST HAVE A REFERRAL FROM TSHC!
5) What if I do not have insurance?
a. Eyerly Ball is the community mental health facility in Ames and they have a sliding fee scale.
b. Other places with sliding fee scales: Kai Lani/Catholic Charities/Ames Therapy and Consulting…
