The Queensland Curriculum and Assessment Authority as delegate for the Australian Skills Quality Authority
About this form
You should complete this form if you are a Queensland school applying to renew your registration
as a registered training organisation (RTO). Please note that this form must be submitted
a minimum of 90 days before the expiry of your organisation’s registration.
There are five sections to this application form:

Unless otherwise indicated, you must provide a response to each question and to each
part of each question.
