FRANKLIN COUNTY JUNIOR FAIR BOARD
4-H BREEDING LIVESTOCK / PET GOAT PROJECT
IDENTIFICATION FORM
A 4-H member must complete this identification form for each breeding livestock project he/she plans to exhibit and show at the Franklin County Fair. The form must include every
breeding animal being cared for by the 4-H member. For example: If a member is raising four breeding lambs for the breeding lamb project, they would record information for all four
lambs on one form. However, if the 4-H member is also taking a breeding hog project those animals would be on a different form. All breeding animals must have some form of
identification. It is the responsibility of the 4-H member’s family to have the animal properly identified with a tattoo, brand, band, ear notch, or ear tag by possession date.
Please refer to the livestock possession information to determine possession date. Please refer to livestock possession information to know what documentation must be submitted with
this form. This form must be postmarked no later than 10 days after the required possession date for each animal and mailed to: FRANKLIN COUNTY JUNIOR FAIR BOARD,
PO BOX 288, HILLIARD, OHIO 43026 or EMAIL to: franklincountyjuniorfair@gmail.com
