Job Description Service CRI Spectrum Hertfordshire Job Title Watford General Hospital Brief Interventions Worker KC1799 Hours 37.5 hrs per week Base Watford General Hospital and Watford CRI Spectrum Salary Range CRI points 26-30 Reports to Hospital Liaison Service Coordinator Purpose of Job To assess patients admitted to hospital using the AUDIT 10 screening tool To provide brief interventions to patients scoring below 20 on AUDIT 10 To promptly refer complex cases and alcohol dependent patients to the substance misuse liaison nurse To work in close partnership with A&E, RAID and Spectrum to manage and develop care plans for frequent hospital attenders To provide a holistic approach to assessing, care planning and care coordination of patients To provide advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways. To refer suitable patients into CRI Spectrum treatment services, including but not exclusively CRI. Develop effective relationships with key stakeholders within the hospitals in Hertfordshire Develop effective relationships with drug and alcohol services in neighbouring counties to enable easy transfer of care post discharge. Ensure that all data requirements for the Watford General Hospital Liaison Service are met within defined deadlines This is a standards based job description that incorporates the National Occupational Standards relevant to the job Key Activities: To provide advice and liaison to all wards and departments. To promote the identification and referral of potential patients for assessment and support within Watford General Hospital. To provide assessment of patients presenting with alcohol issues with in a bio-psycho-social framework. To offer opportunistic and structured brief interventions to clients assessed whilst in hospital. To complete risk management plans in conjunction with primary care, hospital staff and psychiatric liaison for clients presenting with complex risk. To signpost clients whilst in hospital to relevant support agencies. To develop and maintain health promotion resources and related material easily available to all wards and departments. To carry out regular audits as required. To provide a robust data set of clients seen in line with NDTMS data sets and PCT requirements. To continue to develop an effective substance misuse liaison service and optimal screening of clients presenting with an alcohol and/or alcohol related problem in the context of in-patient care. Develop effective relationships with key stakeholders in the hospital, strategy groups and other statutory and voluntary sector agencies in the borough and neighbouring boroughs to operate in a way that promotes collaborative working. Ensure that service user and carer views are incorporated into the development of the service. In conjunction with key stakeholders, to develop and facilitate education/training opportunities to enhance the knowledge and skills of clinical staff working with this group. To participate in multi-disciplinary team meeting/case discussions on the wards/departments. To record assessments, case discussions and therapeutic interventions on the electronic system and hospital case notes where appropriate. To liaise with GP, Community Mental Health Team's (CMHT), Substance Misuse Services, Substance Misuse Care Management, Psychiatric Liaison and inpatients units as required. To effectively communicate with GPs keeping them informed of patients status and care plan, including discharge plan/referrals in a timely manner. Provide information, support and advice and make referrals, as appropriate, to other teams and external specialist agencies, adhering at all times to local service protocols. Keep up to date with interventions being used in the problem-drinking field. Work in partnership with service users, involving them fully in the planning of referrals. To be aware of carers needs and providing advice on carer’s support services. Inform all service users of their rights and responsibilities, advising them about CRI’s policies and procedures, including complaints, protection from harm, equal opportunities and harassment. To respect and work within patient confidentiality. To adhere to CRI and Watford General Hospital policies and protocols. Day to Day Office Management Maintain accurate, up to date records and comply with CRI’s management systems and Watford General Hospital requirements. To provide accurate statistics when requested, which will also be used at the review of the service. Actively contribute to the smooth running and day to day management of the Hospital Liaison Service, through active co-operation with colleagues, teamwork, timeliness and adherence to CRI’s management systems and local service protocols. To attend, prepare for and participate in regular supervision. To attend training and provide feedback as required. To work in partnership with service users and colleagues across the substance misuse field, in order to provide an integrated, relevant and effective service.
