Request Cognos Report from Admin Administrator Print Cognos Report and turn into SAT Lead SAT Lead Enter information into Synergy (SAT Form Syn1) SAT Lead Send out Notice of Invitation to Meeting (SAT Form ITM) to parents SAT At Meeting: Team will review SAT Referral Packet, Parent Case History, documentation and COGNOS report. Complete Intervention Tracking Form (SAT Form ITF) by prescribing interventions, recording minutes and obtaining signatures. (Completed/updated EACH time interventions are prescribed) Schedule next meeting to revisit prescribed interventions and analyze signs of success. Decide whether to complete student observation. If so print Parent Consent for Screening/Observation (SAT Form PC1) and get parent signature. When Parent Consent is returned, give Student Observation (SAT Form SO) to administrator or counselor. After Meeting: Put copy of Intervention Tracking Form in a yellow folder in cumulative folder Give copies to staff who interact with student. If student is an ELL, send copy to Jennifer Haan (jhann@lcps.k12.nm.us) At next scheduled meeting, Referring Party Provide updated student data (behavior logs, anecdotal records) Determine whether to continue interventions or conduct a Functional Behavioral Assessment (FBA) – If so, get parent signature on Parent Consent for Screening/Observation (SAT Form PC1) SAT SAT Behavior Process 1 of 2 SAT At next scheduled meeting, Complete Functional Behavioral Assessment (FBA), if warranted. If Behavior Intervention Plan (BIP) is needed, complete or schedule next meeting. If so, get parent signature on Parent Consent for Screening/Observation (SAT Form PC1) . Make sure parents are invited. Decide whether a psychological screening should be done. If so, get Parent Consent for Screening/Observation (SAT Form PC1) for Referral for Psychological Evaluation and turn in to Dr. Martin Greer. SAT Continue to monitor through SAT If student is on an FBA or BIP, change status of student in Synergy – (SAT Form Syn2) **Depending on severity of behavior, SAT team (including school psychologist and social worker/ counselor) may determine if more immediate steps should be taken towards an FBA or BIP. The school psychologist may override SAT steps in an emergency situation in order to proceed to assessing for eligibility of an emerging exceptionality. SAT Behavior Process 2 of 2
