DIRECTIONS: We used your responses from our first questionnaire to aid us in our initial college
explorations with you. It is now time to turn our attention to how we will relay your unique personality,
interests and characteristics to colleges and universities. Your responses to the following questions will
help us immeasurably as we attempt to capture “your story” in our school recommendations. You will
also find that completing this form will provide you important practice as many of these questions appear
on the college applications you’ll be completing very soon. Please submit this questionnaire to the
College Counseling Dropbox in Haiku by August 30th. We strongly urge you to keep a copy of this for
