All personnel visiting the FIELD UNIT and/or the SURVEY AREA, All temporary hired or contracted vessels (such as crew boats, supply boats, refuelling vessels, etc.), small boats, barges, during their travel time to the SURVEY AREA and their stay and/or stand by on the SURVEY AREA. COMPANY reserves the right to: Suspend the operation at CONTRACTOR's expense in the event of an unsafe practice(s) or condition(s) and/or incident(s) due to non-compliance on the part of the CONTRACTOR with the required Safety and Environment standards under the CONTRACT, Terminate the contract if CONTRACTOR fails to maintain the required Safety and Environmental Standards under the CONTRACT or cause an imminent hazard or serious injury by failing to remedy any unsafe practice or condition previously identified. CONTRACTOR will be required to adhere at all times to the International APPLICABLE LAW (IMO, SOLAS, etc.), the Local Laws and Regulations, the COMPANY and CONTRACTOR Health, Safety and Environment Corporate Policies and to the CONTRACT requirements and procedures concerning Health, Safety and Environment, as described in this EXHIBIT. CONTRACTOR is referred to the Health and Safety Standards as set out in the Safety Manual for Marine Geophysical Field Operations (IAGC Edition), the O.G.P. Health and Safety Schedule for Offshore Geophysical Operations and to the Edition of the IAGC's Environmental Manual for Worldwide Geophysical Operations. CONTRACTOR is also advised to document any pre-existing adverse conditions encountered during previous operations in similar environment. 1.2 COMPANY Specific Requirements A Safety Advisor and/or a Safety Supervisor must be present at all-time onboard the seismic vessel (or master vessel for TZ/OBC operations). Onboard medical and first aid facilities (clinic, equipment, drugs, etc. to be detailed in the Operational HSE Plan) are required as well as victim evacuation capabilities (to be detailed in the Operational HSE Plan: Medevac Plan). COMPANY requires the presence of a Doctor at all times on board the seismic vessel and all the necessary and adequate equipment (Oxygen masks and bottles in sufficient quantity, defibrillator, etc. to be detailed in the Operational HSE Plan), in order to stabilise a victim before a possible evacuation. In the event of multi-vessel operations, such as TZ/OBC seismic acquisition, a certified Paramedic can assist the Doctor. Their skills must comprehend intensive care and good knowledge of anaesthesia techniques. A support vessel to assist the seismic vessel in its operations (chase boat, crew boat, etc.) but also suitable for assistance in case of emergency situation (Medevac). A Vessel Tracking System (such as AVLS) must be installed on all vessels of the fleet in order to monitor vessel’s location on the survey area in particular during operations inside and around Oil field installations. All accidents/incidents shall be investigated, even if not resulting in lost time injury (LTI). Accident reports shall be forwarded to COMPANY REPRESENTATIVE and/or to the onboard COMPANY SUPERVISOR within 24 hours. An accident investigation form Page - 3 BEOCL 10032 EXHIBIT A1-HSE shall be included as an appendix to the Operational Safety Plan. COMPANY reserves the right to take part in any such investigation. 1.3 All safety warnings and safety manuals shall be in English and other relevant languages where local labour is employed. All crewmembers shall have a Medical fitness certificate less than one year old. NB: For all food handling personnel (Cooks and assistants) a medical check certificate less than 3 month old is required. Operational HSE Plan Before Starting Date, in addition to the CONTRACTOR's own Safety Manual which will be available at any time on the Field Unit, CONTRACTOR must submit an “Operational (crew) Health, Safety and Environment (HSE) Plan” to be detailed in EXHIBIT A2 (Particular HSE Specifications). The Operational HSE Plan specific to the considered WORK will be available at any time on the Field Unit. This document will cover, but will not be limited to, the following items: 1.4 Health, Safety and Environment Management System (HSE MS) CONTRACTOR's HSE MS will be included in the "Operational HSE Plan". The following Field Unit Safety characteristics, actions and planning will be detailed: - 1.5 Definition of Line Management Responsibilities including HSE Job descriptions, Management of Sub-Contractors, Personnel Protective equipment (PPE) policy, Training, Auditing, Safety Committee, Meetings (daily, weekly, toolbox, etc.), Drills (fire fighting, Medevac evacuation, MOB, etc.), Incident Reporting , Records (digital data base, paper logs), Communications (Equipment and Contact List) listed in the HSE Emergency Plans including COMPANY's Emergency Plan (if required), Work permit system, Certificates [vessel(s), crane(s), etc.], Milestones, Others. Safety Standards - Specific written procedures for work operations (Operational Safety), Identification of WORK related hazards and preventive measures (Hazard Register Form), PERSONNEL and equipment transportation, Helicopter operations procedures (if any or in case of emergency situation), Helideck certificate(s), equipment and capacity, Safety equipment description and Medevac capability of the support vessel, Marine and Seismic operations procedures (cables and airguns operations, personnel boat to boat transfer, etc.), Small boats operations procedures when working on cable at sea (Barnacles, Tail buoys cross, entangled cables, etc.), Work Boat Safety equipment description, launching and retrieving procedures (Davits system is preferred), Man Over Board (MOB) seismic vessel specific equipment description and procedures, MOB boat Safety equipment, launching and retrieving procedures (Davits system is preferred), Medevac Plan (COMPANY's Emergency Response Plan to be included if necessary), Hazardous materials, Other. Page - 4 - BEOCL 10032 EXHIBIT A1-HSE 1.6 Health Standards CONTRACTOR is required to provide and maintain high standards of hygiene. Areas of concern should include, but not be limited to: - 1.7 Preventive measures and effective treatment for all PERSONNEL (according to local Health requirements), Provisions for potable (drinkable) water, Water Making System for usable (kitchen, shower, etc.) waters: chemical treatment, filtration, analysis, Sanitary toilets and shower facilities, Fumigation (when necessary) for insects invasion, Sleeping quarters for adequate sleep, Rubbish disposal, Kitchen facilities, Food storage and conservation, Adequate air conditioning and heaters according to local climatic conditions, Appropriate clothing according to local climatic conditions, Adequate ventilation of occupied enclosed spaces, Hazardous materials, Other health measures according to local conditions. Environmental Standards In the event an "Environmental Impact Study" has been preliminary conducted by COMPANY, CONTRACTOR must also refer to this document. Areas of concern should include, but not limited to: Sewage and waste disposal (incinerator, treatment, etc.), Hazardous materials, Fuel replenishment and/or refuelling at sea, Environmental Emergency Response Plan (Oil spill contingency Plan), Wildlife reporting Other. 1.8 Health, Safety and Environmental Audits On top of CONTRACTOR’s own Safety audit program (Internal and/or Cross-audits), COMPANY reserves the right to carry out Health, Safety and Environmental external audits as necessary, and CONTRACTOR will co-operate with any such requests. 1.9 1.9.1 Special Conditions Medevac In the event that CONTRACTOR is utilising COMPANY's facilities, as described in COMPANY's Emergency Response Plan, for Medevac purposes, COMPANY will not be held responsible for any incident and/or whatsoever inconvenience arising during victim transit within COMPANY facilities and/or transportation to final destination. 1.9.2 Safety Regulations in Oil and Gas Field Installations When working in, and/or around Oil and gas Field Installations whether belonging to COMPANY or not, CONTRACTOR is required to fully comply with all the Safety and Environment Instructions given by the Oil Field Authority, the Standing Instruction Manuals for Marine Operations (if any) and all the related documents and procedures available with the Oil Field Authority Representative and/or with COMPANY REPRESENTATIVE. In this particular situation CONTRACTOR must observe the following requirements: - Minimum Safety distances to all installations as defined in the Company's oil and gas safety manual and/or specific requirements, Page - 5 - BEOCL 10032 EXHIBIT A1-HSE - Safety inspection and briefing to be conducted before entering or approaching any oil and gas field installations, - Permanent co-ordination with the Company's oil and gas field Logistics and/or Safety departments regarding the operations in progress (ex: diving operations), - Special care and supervision when working close to oil and gas field constructions and pipes, - Operations that involves cutting, welding or burning etc., within the perimeter of any field installations shall not be carried out without obtaining the necessary Work Permit, Any equipment to be installed on oil or gas field installations shall be classified for operation in hazardous areas and shall be operated under the work permit system in place.
