BALTIMORE CITY PUBLIC SCHOOLS GRADUATION MATERIALS ORDERING FORM COMMENCEMENT SERVICES 2013 February 8, 2013 SCHOOL NUMBER The purpose of this document is to outline the process for ordering printed materials and rental equipment for graduation. Funding for these purchases will be covered through the Chief Academic Office (CAO) budget for SY2012-2013. This document contains 5 sections: Middle Schools Only A. Part I- 8th Grade Certificate Data B. C. D. E. High Schools Only Part II- Graduation Coordinator Contact Data Part III - Printed Graduation Materials Part IV - Verbiage for Graduation Tickets and Announcements Part V- Equipment Rental School Instructions: Schools need to return this form as an email attachment to Patricia Harris (PGHarris@bcps.k12.md.us) by March 1, 2013. Please note the following: 1. All items requested will be delivered to the schools on or about the April-May time frame. The exact date is dependent upon the timely completion of the form by schools. 2. Additional items or changes requested to this ordering form will not be accepted. 3. Schools opting to print their own Commencement programs need to use the proper “Official Page” for City Schools. To obtain a copy of the official page, please contact Patricia Harris at PGHarris@bcps.k12.md.us. 4. This list will be sent to a print shop to be inserted into the Commencement Program template draft that will be sent to the above Coordinator at a later date for editing, at which time non-graduates can be removed from the draft template.
