RELEASE AND LIABILITY WAIVER
Covering Saturday, July 25 – Friday, July 31, 2015
If you are 18 years old or older, you must read and sign the following waiver before attending the Building
Tomorrow’s Missions trip sponsored by the First Baptist Church of Clinton, LA in association with Santa Teresa
Baptist Church of Santa Teresa, NM.
If you are under 18 years of age, a parent or guardian must read and sign the following waiver before doing
volunteer work for the Building Tomorrow’s Mission trip sponsored by the First Baptist Church of Clinton, LA in
association with Santa Teresa Baptist Church of Santa Teresa, NM.

The Volunteer desires to work as a volunteer for the First Baptist Church and/or the Santa Teresa Baptist Church of
Santa Teresa, NM and engage in the activities related to being a volunteer (the “Activities”). The Volunteer
understands that the Activities may include travel to and from the mission, assisting in VBS, or if traveling to
Juarez, work on the buildings.
The Volunteer hereby freely, voluntarily, and without duress executes this Release under the following
terms:
1. Release and Waiver. Volunteer does hereby release and forever discharge and hold harmless the First Baptist
Church of Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM, and their successors and assigns
from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or
may hereafter arise from Volunteer’s Activities on the STJ Mission. Volunteer understands that this Release
discharges the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM from
any liability or claim that the Volunteer may have against the First Baptist Church of Clinton, LA and/or Santa
Teresa Baptist Church of Santa Teresa, NM with respect to any bodily injury, personal injury, illness, death, or
property damage that may result from Volunteer’s Activities with the First Baptist Church of Clinton, LA and/or
Santa Teresa Baptist Church of Santa Teresa, NM, whether caused by the negligence of the First Baptist Church of
Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM or its officers, directors, employees, or agents
or otherwise. Volunteer also understands that the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist
Church of Santa Teresa, NM does not assume any responsibility for, or obligation to, provide financial assistance or
other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness.
2. Medical Treatment. Volunteer does hereby release and forever discharge the First Baptist Church of Clinton, LA
and/or Santa Teresa Baptist Church of Santa Teresa, NM from any claim whatsoever which arises or may hereafter
arise on account of any first aid, treatment, or service rendered in connection with the Volunteer’s Activities with
the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM.

3. Assumption of the Risk. The Volunteer understands that the Activities may include work that can be
hazardous to the Volunteer, including, but not limited to, construction, loading and unloading, and
transportation to and from the work site, and that food, accommodation, and medical facilities may be
donated and beyond the control of the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist Church of
Santa Teresa, NM. Volunteer hereby expressly and specifically assumes the risk of injury or harm in these activities
and releases the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM from
all liability for injury, illness, death or property damage resulting from the Activities of the Volunteer’s participation
with the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist Church of Santa Teresa, NM.
4. Insurance. The Volunteer understands that, except as otherwise agreed to by the First Baptist Church of Clinton,
LA and/or Santa Teresa Baptist Church of Santa Teresa, NM in writing, the First Baptist Church of Clinton, LA
and/or Santa Teresa Baptist Church of Santa Teresa, NM is not obligated to and does not carry or maintain health,
medical, or disability insurance coverage for any Volunteer.
Each Volunteer is expected and encouraged to arrive with medical or health insurance
coverage in effect.
5. Photographic Release. Volunteer does hereby grant and convey unto the First Baptist Church of Clinton, LA
and/or Santa Teresa Baptist Church of Santa Teresa, NM all right, title and interest in any and all photographic
images and video or audio recordings made by the First Baptist Church of Clinton, LA and/or Santa Teresa Baptist
Church of Santa Teresa, NM during the Volunteer’s Activities with the First Baptist Church of Clinton, LA /or
Santa Teresa Baptist Church of Santa Teresa, NM, including any photographs or recordings.
6. Other. Volunteer expressly agrees that this Release is intended to be as broad and inclusive as permitted
by the laws of the State of New Mexico, and that this Release shall be governed by and interpreted in
accordance with the laws of New Mexico. Volunteer agrees that in the event that any clause or
provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of
such clause or provision shall not otherwise affect the remaining provisions of this Release which shall
continue to be enforceable.

Churches may send the forms for their entire TEAM in one packet. Beware that there is a maximum of
40 workers that can be housed at the Santa Teresa Baptist Church.
