Pharmacist We are seeking one part time Pharmacist for one day of service per week that is a leader and change agent for this new model of care. We are a growing clinic and the environment is one of dynamic change. Our team members must have a willingness to face challenges as they arise, productively resolve conflicts and provide creative solutions that enable us to continue our commitment to our vision of “Improving Health Together” as well as our Team Charter. The Pharmacist brings demonstrated leadership qualifications or leadership experience to the team. The Pharmacist promotes customer service first and works with the team to enhance access to care. The Pharmacist is prepared to work a flexible work schedule to meet the needs of the clinic and the needs of the community. Solutionfocused problem solving and conflict resolution is the expectation for all team members. The Pharmacist participates fully in continuous quality improvement, self-improvement, team development, community development, health links and fostering community partnerships. Lifelong learning is expected and supported. The Pharmacist utilizes a variety of evidence-based treatment approaches in caring for individuals and families across the lifespan. The Pharmacist has a sound knowledge in the principles and practices of prescribing for seniors. Ideally, the Pharmacist is certified as a Diabetes Educator (or is willing to achieve certification) or has Training Enhancement in Applied Cessation Counselling (TEACH) and will co-manage the care of individuals with diabetes and other chronic illnesses or disease. The Pharmacist will be actively involved in the safe prescribing of medications and will be a consultant to our clinicians and physicians. The Pharmacist will see clients through individual or group appointments. The Pharmacist may be involved in establishing programs or participating in programs for our clients including our annual influenza vaccination program. The Pharmacist will manage samples and coordinate educational updates for our clinical team. The Pharmacist has graduated from a recognized University program. He/she is licenced with of the Ontario College of Pharmacist’s and is in good standing with the College. Three-five years' experience in a team-based model of care is preferred. Computer skills are essential. Current immunization, Accessibility Training, WHMIS and Mask Fit testing are required and supported by the employer if not in place. Vulnerable sector police clearance is required. Current Ontario Driver's Licence with insurance for business travel is required. Additional certification is an asset.
