ALL Parent Volunteers MUST have clearance with the Broward County.

6TH – 8TH
9TH -12TH
8:30 am – 10:30 am
11:00 am – 1:00 pm
1:00 pm – 2:50 pm Food Trucks ONLY
(Bring monies on the day of event)
SACM STUDENT FUN TICKET PRICE:
$20.00 LUNCH INCLUDED (see reverse side for more Fun Ticket details)
PAYMENT: CASH, MONEY ORDER, CASHIER CHECKS ONLY. WE DO NOT ACCEPT CHECKS.
FIELD TRIP FORM/INSURANCE FORMS DUE DATE: January 29, 2016 after this date the cost will got
up to $25.00 from February 1-12, 2016. If you bring money on the day of this event all students must
have their permission slip, insurance form and “Fun Ticket Form” filled out…
All students attending must have insurance. If they have not submitted insurance for previous field trips they must
provide their field trip insurance application with PROOF of Insurance at this time; without insurance they
cannot attend any field trips.

Parents and Siblings are welcome and Fun Ticket can be purchased on the day of the
event for $25.00.
 Food is available for purchase on the day of the event from
 Food trucks, Philly Italian Ices.
