2015 AAU Girls Basketball Grade Verification Form
This form must be completed and attached to each player's birth certificate. An administrator of
the player's school must sign the form. This form must be carried to every Sanctioned TN Girls
Basketball event. It will only be needed if a player's grade and age is protested. If a protest is
brought up and the this completed form is not available upon request, the forfeit of the game
and tournament will be honored by TN Girls AAU Basketball
NOTE: All information is necessary for proper identification. Player will be rejected if information is incomplete.
