Please send the Pro-Vice-Chancellor Variation Approval Form, along with a copy of the student’s academic record and any other relevant documentation, to the Health Sciences Divisional Office, attention the Academic Manager. Documents will be signed by the Pro-Vice-Chancellor’s nominee, the Associate Dean Academic, each Friday afternoon. All documents for signing must be received by noon on Wednesday to be signed by the Friday of the same week. Once a decision has been made, a copy of the signed form will be returned to you.
