District Access to School Psychologist Behavior Assessment Services Currently IEP Eligible Student New Referral Student District Personnel completes School Psychologist Assessment Request & forwards copy of Review of Existing Data to ACES Assessment Team for review 5 days prior to meeting District requests ACES Assessment Team consultation & observation prior to beginning the RED or eligibility process ACES Assessment Team reviews Assessment Request and RED to determine if additional assessments are needed Additional Assessments NOT Necessary Additional Assessments Necessary ACES Assessment Team consults with District Personnel, provides consultation, observation and support as needed ACES Assessment Team consults with District Personnel & makes referral to School Psychologist for completion of Behavior Assessment Referral Forms: http://www.aces.maryville.k12.mo.us/ forms.html Evaluation NOT Necessary, or additional data needed prior to referral Evaluation Necessary, data is sufficient Interventions provided & referral to Social Worker (Minimum 6-8 weeks) District Personnel completes School Psychologist Assessment Request & forwards copy of Review of Existing Data to ACES Assessment Team for review 5 days prior to meeting ACES Assessment Team continues consultation with School Psychologist, Social Worker and District Personnel to determine if additional consultation hours are needed ACES Assessment Team reviews Assessment Request and RED to determine if additional assessments are needed Additional Assessments NOT Necessary ACES Assessment Team consults with District Personnel, provides observation as needed Additional Assessments Necessary ACES Assessment Team consults with District Personnel & makes referral to School Psychologist for completion of Behavior Assessment
