Amendments
There are 7 workstreams – not 9 as noted in previous mins.
It was stated that localities will not be represented on workstreams.
Overview of workstreams as attached with these minutes.
Integration scheme consultation
It has been noted that there is nothing in scheme to reflect Surgical Specialities.
Feedback from the consultation will be published online. Process should take till beginning of April.
Deadline tomorrow Friday 6th March 2015. A lot of comments received so far.
Locality Managers Post
The Council are taking the lead with this. No dates set as yet for interviews. Closing date was
27.02.2015. Waiting for assessment centres being set up.
Current Locality Co-ordinators posts to be extended till end of June 2015 to support localities until
Managers appointed.

Jeff Ace has agreed to attend the next meeting.
Could the group please have a think about any questions and/or specific issues they would like to

Communication Graduate for Integration
Interviews have not yet been held. There are 14 applicants for the post.
Strategic Planning – Ann Stevenson
Ann is one of 4 Joint Planning Commissioning Managers in post who work with Vicky Freeman and
the team at Strategic Planning. Ann will provide support to the Annandale & Eskdale Locality and
provide a link between Locality and Strategic Planning.
It was agreed that it would make sense for the 4 Locality areas to have a common template for their
Locality plans. Each plan will be area specific in it’s own right but the common template will make
for easy reading and collation. Ann and the other Joint Planning Commissioning Managers are
meeting next week to look at the template. Given the timescale there was a request for this to be
sent for comment asap.
Health intelligence and mapping work carried out – will contribute to plan.
Ann, Elaine, Robyn and John met last week to look at work already completed (eg survey sand
roadshows ) and how we use this to inform the plan.
Social Work input
Jimmy highlighted that the Social Work team locally had set up a local integration group to look the
9 outcomes and given their thoughts and ideas. This is a useful process, creating awareness and
ownership
Locality Plan Timescale
Locally we will be required to be fully consulted and completed by 31.03.2016. We need to allow at
least 6 months for consultation so really this gives us till the end of September 2015 to work at this.
Concerns were raised that without a Locality Manager still not in post we may need to identify
capacity to assist in writing/pulling the plan together.
We need to focus on some of the big issues / challenges but also remember the opportunities and
things that have worked well. John had previously requested feedback on what people considered
to be the big issues, no feedback to date – Faye to send out email to prompt.
Question was raised whether the Locality plan will be for 1 year or 3 years? It was agreed that the
plan would be an evolving document. The first year will be about developing a solid base from which
to build.
Readiness for Integration Toolkit
This has been circulated to the group. John Patterson from the JIT would like to attend an event with
localities and get feedback on this document.
Await date for this – it was requested to be sooner rather than later if this was to assist in the
planning process– hopefully April time.
Forward Looking Care
Final report will be ready at the end of March. We need to consider this in locality planning and what
approach we would take to roll this out. Elaine and Donna attended Locality Planning meeting in
Wigtownshire to talk about the FLC project. This was received positively.
Staff Engagement
Leadership Development Programme – delegates identified from across range of services. Starts in
April.
Talking Points Training is scheduled for 25th & 26th March. One day set for managers/supervisors and
one for practitioners.

Public / Community Engagement
Community survey under way. Partners in tne Third sector are supporting this work. Target to speak
to 2000 people. Spending quality time to helping to get more in depth feedback.
It was agreed that the integration leaflet and survey monkey link to sent to staff in A&E to: Raise awareness about the survey/consultation process
 Encourage teams to ask/support their clients to complete a survey
 Take part in the survey if they live in the locality.
Area Committee Seminar
A seminar is being arranged for elected members. To be held in April 2015. Initially the request was
for a focus on Day Centres but it was agreed to widen the scope.
The seminar will look at what we do collectively to support vulnerable people in communities and
particularly in terms of Health & Social Care Integration.
It was highlighted that there are 10 Day Centres across the region; 4 of which are in Annandale &
Eskdale. Graham Abrines has presented a paper committee and funding has been found for the next
year from P YF funds. It was raised that future Day Centre Funding may be required to come from
Locality funds. Questions were asked about whether this funding be devolved or will the Localities
be expected to pick up the tab?
Day Centres have been asked to look at outcomes in relation to users.
£471,000 has been given to support Day Centres across the region.
Communication
Looking to develop joint website – a stand alone central portal for information.
New hospital / change links and integration.
Each Locality to have a section for their own input.
Finance
A&E have been allocated £258,000 for 1 year to support the H&SCI process
A pro forma will be produced to help localities to plan and ensure all activity and spend is linked to
outcomes.
The other £2 million will sit regionally and will support the workstreams and Programme Board.
Some projects that span across the region may apply to the Regional pot for funding.
Some ideas raised for funding..
Community Link Workers – keen to progress.
Co-location. Develop Hubs.
Forward looking care planning – develop and roll out across Annandale & Eskdale.
Promote public responsibility of own care.
Time banking – focus on supporting delayed discharges.
Care package reviews – how we replace/amend current packages with different thinking. Having
somebody dedicated to do this.
Social Work referrals – changing peoples expectations and streamlining processes.
Bed capacity.
Scope / skills for carers – managing simple and common health care needs.
Information sharing and recording across care homes. E-Pen?
Falls training and prevention.
Better use of Telecare technology.
Effectively engaging with GP Practices.
Prescribing –raising awareness for both patients and staff.
Managing ordering, stocks and finances more effectively and efficiently.
RVS Transport Service - If each Locality contributed £6,200 the service could keep running

Expecting end of test reports – to be submitted by tomorrow, Friday 6th March 2015.
Link to website sent out – information about all projects on the site.
Data should provide assistance when making decisions where to utilise finds.

Donna Wallace – returning to day job (Registered Nurse in Annan District Nursing Team) but will
now have x1 day dedicated to Forward Looking Care permanently. Funding has been arranged for
this.
