Procedures for Service Animals in Schools
Anderson School District Five does not permit discrimination against individuals with
disabilities, including those who require the assistance of a service animal. The district
will comply with all federal laws regarding the use of service animals under appropriate
circumstances.
Definition: Service Animal means any guide dog, signal dog, or other animal
individually trained to do work or perform tasks for the benefit of an individual with
a disability.
Guidelines: 1. An individual with a disability may submit a written request to the
Director of Special Education to bring a service animal to school for educational
purposes. (See attached Request for Use of Service Animal form.)
2. The school district’s review of a request for the use of a service animal
may include consideration of a student’s special education IEP team or the student’s
Section 504 team.
3. The use of a service animal on school district property is subject to a plan
which introduces the service animal to the school environment, any appropriate training
for staff and students regarding interactions with the service animal, and other activities
as deemed necessary by the school district.
4. The school district’s approval of the use of a service animal on district
property is subject to periodic review of the conditions and circumstances in which
the service animal is being used.
5. Service animals must wear proper identification and always be on a leash
or other form of restraint mechanism. It is the responsibility of the student who uses a
service animal to properly handle and maintain control of the animal. Any cost incurred
to handle the service animal will be the responsibility of the parent of the student who
uses the service animal (or the student if the student has reached Age of Majority).
6. The Owner of a service animal must provide annual proof of the
following vaccinations: DHLPPC (Distemper, Hepatitis, Leptospirosis,
Paroinfluenza, Parvovirus, and Coronavirus), Bordetella, and Rabies.
7. All service animals must be treated for and kept free of fleas and ticks.
8. All service animals must be kept clean and groomed to avoid shedding
and dander.
9. Service animals will be allowed in school district transportation
vehicles only when:
 It has been determined through the IEP process that the service animal is
a necessary part of a transportation accommodation; 

 The inclusion of the service animal is documented in the IEP; 

 The service animal is under the control of its handler at all times
including entering and exiting the vehicle. 
10. The school district retains discretion to exclude or remove a service
animal from its property if:
 The animal is out of control and/or the animal’s handler does not
effectively control the animal’s behavior; 

 The animal is not housebroken; 

 The animal poses a direct threat to the health or safety of others that
cannot be eliminated by reasonable modifications. 

 The animal’s presence or behavior fundamentally interferes with
the functions of the school. 
Liability: Owners of service animals are liable for any harm or injury caused by the
animal to other students, staff, visitors, and/or property. The owner will hold the school
district harmless and indemnify the school district from any such damages.

SERVICE ANIMAL AGREEMENT
I have read and understand the School District’s Service Animals Procedure. I will abide
by the terms of the Procedures.
I understand that if my Service Animal is: out of control and/or the animal’s handler
does not effectively control the animal’s behavior; not housebroken or the animal’s
presence or behavior fundamentally interferes in the functions of the School District; or,
poses a direct threat to the health or safety of others that cannot be eliminated by
reasonable modifications, the School District has the discretion to exclude or remove my
Service Animal from its property.
I agree to be responsible for any and all damage to School District property, personal
property, and any injuries to individuals caused by my Service Animal. I agree to
indemnify, defend and hold harmless Anderson School District Five from and against
any and all claims, actions, suits, judgments and demands brought by any party arising on
account of, or in connection with, any activity of or damage caused by my Service
Animal.

Note: This Registration/Agreement is valid until the end of the current school year. It must be renewed
prior to the start of each subsequent school year or whenever a different Service Animal will be used.
