To increase capacity in primary and secondary care and reduce the unnecessary A&E, Walk-in Centre and GP activity through development of a prescribing pharmacist service and in order to improve patient experience and access to services closer to home. STH/UC1 In hospital care – urgent care STH/UC2 In hospital care To reduce pressure on urgent care services and enable patients to be supported to remain within their own residence promoting independence and reducing unnecessary admissions with a focus on improving quality and patient safety through the implementation of a GP support service in and out of hours for NEAS paramedics. To work with Local Authority partners to deliver an integrated Advocacy Hub to respond to the challenges and meet the quality requirements of the Winterbourne View concordat improving access to advocacy support particularly within residential units. Ensuring that the community is sufficiently resourced and equipped to deliver robust health care to individuals with complex needs to prevent re-admission and facilitate effective discharge for those people that have received periods of assessment or treatment intervention To expand the Cataract Enhanced Service into the Stockton Locality to ensure access to equitable services across Hartlepool and Stockton-on-Tees and improve patient choice and reduce unnecessary referrals to secondary care. To improve the health and wellbeing of the frail and elderly population living in Hartlepool and Stockton, by providing a proactive, structured approach to care through implementation of a number of schemes to include: STH/OHC2 STH/OHC4 Out of Hospital Care Care Homes End of Life And to develop a Frail Elderly Care Strategy to deliver improvements over the next 5 years To develop Primary Care Schemes to improve GP access and reduce demand on urgent care services.
