Dear Mr. Gayoso,
Please be informed that our child, ___________________________, who is part of the
village delegation to _____________________, will not be travelling back to the Philippines
with the delegation.

______________________ and from hereon, our child will no longer be the responsibility of
his/her Adult Leader or CISV Philippines.
We understand that our child [full name of participant] is leaving early from
the itinerary of his/her delegation. As a result, he/she will be travelling with us
from ______________ to the Philippines. We understand that our child's travel insurance
that we have procured through CISV Philippines is valid for only 33 days after the
date of departure from the Philippines. If our child's stay in (Europe/USA/other)
will go beyond the validity of the aforementioned insurance policy, we will be
responsible for procuring additional travel insurance.
We waive all rights and claims of damages of any kind or nature whatsoever
which we may have against CISV International Ltd., CISV Philippines Association,
Inc., and CISV – Quezon City Chapter and their authorized representatives, arising
out of any events that may occur after the early turnover of our child to us on
____________ .
We have signed this legal document on the date stated immediately below as
proof that we consent to the early turnover of our child to us on _______________.
