At least one reference contact (name, phone number, and email address) who can verify the Alum’s project and its impact. Student’s photo 1: head shot Student’s photo 2: performing service Answers to the following essay questions (in one document): o In 300 words or less, describe the service project(s) you completed for this application. Explain how you learned of this community need/issue and how your service made a quantitative measurable impact (e.g. number of people assisted, amount of money raised, or any other objective quantifiable outcome or measurable results). o In 200 words or less, explain how you have demonstrated leadership in completing your service project. o In 200 words or less, explain how HOBY inspired your commitment to service and leadership. Upon receipt of this verification, HOBY will mail the scholarship check to the Alumni, but the check will be made out to the college.
