Guidelines for completing an application
All proposals should include all of the following details.
Section 1 – Proposed Service
1.1 Outline of proposed service
Include details of:









General area to be serviced
Route description (including terrain, road surface(s), suitability of turning areas)
Special features (including the likelihood of seasonal flooding)
Current route distance (from first pick-up point to school) and how it has been verified (e.g. online mapping
or driving the route)
Proposed route distance (from first pick-up point to school) and how it has been verified (e.g. online
mapping or driving the route)
Adequacy of the existing vehicle to service the needs of students (if applicable)
Benefits of the proposed service (including the nature of the benefits and the number of students who will
benefit)
Drawbacks of the proposed service (including the nature of the drawbacks and the number of students who
will be disadvantaged)
List any sections of the route that may present temporary or seasonal difficulties for use by a school bus.
1.2 Special Requirements
Include (where appropriate) details of:


Special requirements (including conversion for transport of physically disabled students)
Any special reasons why the normal standard for service provision should not apply.
Section 2 - Map
Details of Proposed Service
Enclose two maps: one map is to show the existing services and, a second map indicating:








Proposed route variation (coloured RED)
Present school bus route servicing the school (coloured BLUE)
Existing school bus routes in the vicinity of the proposed route variation (dotted BLUE)
School bus route(s) servicing nearby schools in the vicinity of the proposed route variation (coloured GREEN)
Existing public transport routes in the vicinity of the proposed service (coloured BROWN)
Location of the coordinating school (BLUE dot)
Location of nearby schools (GREEN dot)
Place of residence of each student to benefit from the proposed route variation (YELLOW dots numbered
according to the requirements of Section 3).
Note: the proposed map, unless provided with student addresses will not be processed and will be returned to the
school for further work. PTV may assist in providing current route maps as needed.

Section 3 - Current passenger data
For each student who is affected by the variation to the service if it becomes available, the following data is
required:






Student's surname and initial(s)
School attended
Year level
Distance of the student’s place of residence from both the nearest appropriate school and school attended
Distance of the student’s place of residence from the nearest existing bus route and the proposed bus route
Whether the student will benefit from the introduction of the service variation.
Note: Each student listed should be given a number (consecutive from 1) which should correspond with the
numbered markers on the map required for Section 2 above.
Section 4 - Future passenger usage
For each student who would (within the next five years) use the service, the following data is required:






Student's surname and initial(s)
School to be attended
Year of entry to school
Distance of the student’s place of residence from both the nearest appropriate school and school to be
attended
Distance of the student’s place of residence from the nearest existing bus route and the proposed bus route
Whether the student will benefit from the introduction of the service variation.
Note: Any student listed in Section 3 should not be included in this section.
