Job Description
Health and Wellness Coordinator, Carter Burden/Leonard Covello Senior Program
Qualifications
BA in community health, public health or related field. At least one year experience working with older
adults. Bi-lingual English/Spanish a must. Computer literate.
Organize and direct programs to achieve the goal of the best mental and physical health possible for
participants of the Carter Burden /Leonard Covello Senior Program (Covello Program). Ability to
educate and inspire senior participants to recognize the rewards of good health and work toward
achieving those goals is necessary. Good public speaking skills and ability to interact with public and
private officials and organizations required.
Planning/Evaluating
Develop plans to continue and/or re-design existing programs and to implement new programs
Assess overall physical needs of the Covello Program participants and develop initiatives to address
needs.
Review and evaluate delivery of wellness programs and extent to which goals are being met.
Participate in training for development of evidence based evaluation of health and wellness program(s).
Research community networks and resources available to participants in order to access these and make
referrals on behalf of participants.
Essential Duties
Identify and address risk factors and barriers to participation in health and wellness programs and make
any necessary internal or external referrals for services.
Organize and prepare and deliver and/or hire consultants for workshops, trainings, and educational
materials.
Work at both Covello Senior Program and the Metro East 99th Street Demonstration Model Adult Day
Program
Work with community organizations, public officials, hospitals, to develop community events related to
improving health and enhancing well-being.
Actively market the health and wellness programs available at the Covello Program
Develop leadership skills among senior members so that they can help their peers engage in programs that
enhance their health.
Supervise, provide training to and evaluate the Activities Coordinator
General
Participate in supervisory meetings with the Director of the Metro East 99th Street Demonstration Modal
Adult Day Program.
Keep up-to-date on current trends in wellness and senior center programming
Exhibit ability to work well within agency and engage appropriately with supervisee, co-workers and
administrative staff.
Accept other assignments, as appropriate, to meet the needs of the Covello Program and individual clients
