Duties and Responsibilities:

Prepare and deliver and discontinue hemodialysis treatment as per SEHA Dialysis Services policies and procedure.

Management of vascular access.

Take laboratory samples if needed using appropriate collection methods and labeling.

Recognizes reports and documents early signs of patient’s adverse response to dialysis and take preventive measures where appropriate.

Dissembling, disposes, cleaning off equipment as per infection control policies.

 To participate in an “out of hours on call” Rota.

Unit Overview: The Haemodialysis unit consists of 56 stations on a 3 shifts basis from Saturday to

Thursday. We deal with both acute and chronic renal patients. Dialysis machines are Fresenius 5008S.

Clinical Practice

Responsible for the assessment, planning, implementation and evaluation of patient care in accordance with the organizations policy and procedures and the unit scope of service.

Assumes accountability and responsibility for own practice and does not carry out tasks beyond level of competence.

Utilizes evidence based practice in the provision of care

Coordinates individualized care from point of first contact throughout the episode of care, incorporating discharge planning in the plan of care.

Identifies significant changes in patient conditions, initiates appropriate action and reports to immediate supervisor or physician as appropriate.

Prioritizes patient care based on ongoing assessment of patient needs

Participates in the referral process and utilizes available resources for the provision of comprehensive care to patients and families as per organizational policy.

 Demonstrates service excellence and acts as a patient advocate to support the patient’s rights

Communication

Maintains confidentiality and security of patient, family, and the organisation information in accordance with the confidentiality policy.

Responsible for documentation and participates in the use of information technology and relevant electronic medical records.

Facilitates effective communication between the patient, family and the interdisciplinary health care team as required.

Liaises with other health care providers as necessary to ensure appropriate and timely interventions for patients and for discharge planning.

Actively participates in meetings, rounds and case conferences in the role of primary nurse.

Provides comprehensive clinical handover and reports as required to ensure patient safety.

Ensures that nursing documentation is compliant with organization policies and procedures.

Ensures documentation reflects a continuous, systematic and accurate nursing assessment of all

 patient care events.

Unit Overview: The hemodialysis unit consists of 31 stations for out-patients and 8 stations for in- patients and operates on 4 shifts from Monday to Saturday and Sundays for acute patients only.Dialysis machine are Fresenius 4008S and 4008H.

1.

Technical Function

Explains hemodialysis procedure did pre and post dialysis assessment, monitored vital signs and weight of the patient.

Performs proper cannulation and applied aseptic technique during the procedure.

Monitors blood pressure strictly every 30 minutes and attends to complications that arise.

Ensures that dialysis prescription is carried out, checked good patency of access,

Extracts pre and post dialysis blood examinations.

Monitors and observes patient for possible complication during the whole treatment.

Provides patients comfort and assisted them in their personal needs.

Assist doctors and co-staff during Cardio-Pulmonary Resuscitation and other emergency situations.

Reminds patients about their care of access, home medications and schedule of next dialysis treatment.

Documents all observations done during the hemodialysis treatment.

Coordinative/Facilitative

Reports minor/major machine breakdown, problems on supplies and other equipments used in the area.

Discuss problems with co-workers as well as patients complaints to head nurse and unit supervisor.

Participates in stock maintenance, proper care of equipments and unit cleanliness.

As Charge Nurse ,motivates co-staff to achieve unit objectives, works independently in order to gain trust and loyalty.

Attends in-service training program of the Nursing in Service Training and Research and applies what is learned in as much as patient care is concerned.
