Knights of Columbus Council #15625 St. Michael’s Catholic Community 2014 Jay Henry Memorial Scholarship Application Introduction The St. Michael’s Catholic Community Knights of Columbus Council #15625 is proud to announce the 2014 college scholarship competition. The competition is open to military children of St. Michael’s Catholic Community who will attend college as an undergraduate in the upcoming 2014-15 academic year. This year the council offers 4 scholarships in the amount of $500 each. Essay Theme The Knights of Columbus are driven by four core principles: Charity, Unity, Fraternity and Patriotism. Choose at least 2 of the principles and write an essay no more than 1,000 words on what those principles mean to you and how you practice those principles in your life. Instructions Fill out the cover sheet (page 3 of this document), and complete the Supplemental Information Sheet (page 4 of this document). Staple or binder clip the following items (in order): 1. Cover sheet. 2. Two Letters of Recommendation from current teacher(s), coach(es), school board member(s) or school administrator(s). 3. Supplemental Information Sheet (up to 2 pages total). 4. Your typed essay. 5. Supporting documentation – Include certified (they can be unofficial, as long as they are verified by a school official) copies of your current academic transcripts. (If in college, include a copy of your college transcripts; if in high-school, include a copy of your high-school transcripts), letters verifying leadership, extracurricular activities, and community involvement. All materials must be unfolded and submitted in a 9 X 12 envelope postmarked no later than 13 June 2014. Failure to follow the instructions may result in disqualification. Documentation may include, but is not limited to, the following: letters from organization leaders, proof of participation in activities, certificates or awards with your name on them. Supporting Documentation - Academic Performance (Transcripts): Include current, certified copies of transcripts (they can be unofficial as long as they are verified by a school official) in your packet. If still in high school or a freshman in college, include a copy of your high school transcripts. If you have completed at least your first year of college, include a copy of all college transcripts. Essay: Include your essay in your packet. Put your name in the header of each page. On separate sheets of paper, type a double-spaced original essay of no more than 1,000 words on the essay theme listed on the first page in the “essay theme” section. Important Candidate Information Ensure all of your materials reflect your best work. Make sure you proof read all materials before submitting your packet. Before submitting your packet, it is a good idea to read your information out loud. Have someone else review your work for clarity and to identify mistakes. Make sure the submission is grammatically correct, free of spelling errors, and that details are accurate. If you are selected to receive a scholarship, you will be required to provide verification of military dependent status and college enrollment.
