Each of us has a unique sense of who we are, our conception of self or identity. Below are several statements which people may use to describe their sense of self. Using the rating scale provided, rate the extent to which you agree with each statement as being accurate in describing your sense of self.

There are no right or wrong answers and you are requested to answer on the basis of your own experiences and beliefs, not just on the basis of logic. Take your time and feel free to go back and change your answers. If you have trouble deciding any of these, please make your best choice and do not leave any unanswered.
