If you are interested in trying out for the Junior High or Biloxi High School Cheerleading Squad, a
parent/legal guardian and the student must attend the informational meeting that will explain the details concerning
tryouts. No other relative may substitute.
Candidates must attend workshops in order to learn the required material for tryouts.

 Signed (by parent and applicant) PARTICIPATION WAIVER form *AVAILABLE AT MEETING
 Signed (by parent) EMERGENCY MEDICAL AUTHORIZATION form *AVAILABLE AT MEETING
 Proof of GPA (FALL SEMESTER REPORT CARD – NO I IOD)
 Proof of MEDICAL EXAM (PHYSICAL) or release from personal physician – must include legible exam date. (If
the student has one on file in the Athletic Office, it must be on the list provided by the AD office at the parent
meeting.) The exam must have been given after April 1, 2014.)
 Proof of MEDICAL INSURANCE (A photocopy of the insurance card is required.)
 MHSAA Concussion information form



Don’t wait until last minute to schedule a medical exam (physical). This information is being distributed to give you advance information so you
can meet deadlines.
Applications will not be accepted at the meeting.
Birth Date Requirements:

A 7th grader cannot be 14 before August 1st and compete with the 7th graders
A 8th grader cannot be 15 before August 1st and compete with the 8th graders
A 9th grader cannot be 16 before August 1st and compete with the 9th graders
A senior cannot be 19 before August 1st and compete
