Job Title: COMMUNITY PROGRAM COORDINATOR: PRECONCEPTION HEALTH CAMPAIGN (One position in Jackson County, one in Josephine County) Summary Job Description: The primary function of this position is to coordinate grantfunded, three-year, Preconception Health Campaigns in Jackson and Josephine Counties. The Coordinators will work closely with the Project Manager and a Perinatal Task Force made up of Public Health Departments, Coordinated Care Organizations, Community Health Centers, a Regional Health Equity Coalition, and other key community partners in the medical, mental health, and education fields. Working through this consortium, the program coordinators will organize and oversee the day-to-day tasks involved in successful implementation of community-wide clinical Preconception Health preventive services, and a community-based Preconception Health education and marketing campaign. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Assist in the development, coordination, and implementation of the grant-funded, Preconception Health Campaign, including selection of evidence-based program materials and programmatic approaches, program record keeping, data collection and evaluation, and grant reporting. • Assist in the mobilization, recruitment and retention of Perinatal Task Force members and other key community contacts. May facilitate meetings, identify task force interest, strengths and capacity to engage with the Preconception Health campaign. Channel Task Force members to the best opportunities to plug into the campaign. • Work closely with Community Health Centers, Public Health Departments, Primary Care Clinics, Coordinated Care Organizations, and School-Based Health clinics to implement preconception health screening, counseling and interventions, and embed protocols in electronic medical records. • Work with clinical staff and community health workers to ensure that ongoing training is provided in preconception health screening, counseling and interventions. • Work with Perinatal Task Force to develop and disseminate community-wide Preconception Health education toolkits and materials. • Work with schools and school-based clinics to develop and implement appropriate school-based preconception health campaigns. • Work with Regional Health Equity Coalition to ensure that culturally and linguistically appropriate outreach occurs to the Latino Community. Experience/Skills: • Experience working in health field and promoting health behavior messages • Experience in creating partnerships and building coalitions • Highly motivated with ability to work independently and as part of an interdisciplinary team • Strong people and networking skills required, including ability to work effectively with health professionals, school officials, and community leaders • Excellent verbal and written communication, problem-solving, and organizational skills • Microsoft Office computer skills • Strong commitment to maternal and child health issues • Experience in project management, volunteer development, maternal/infant health, reproductive health and/or youth populations a plus Education: Degree in health education, public health, nursing or related field preferred. Supervisor: Program Manager, Health Care Coalition of Southern Oregon Employment status: Jackson County Coordinator – Three-quarters Time to Full Time (0.75 FTE–1.0 FTE) with pro-rated benefits package. Josephine County Coordinator – Half-time (0.5 FTE), with prorated benefits package. Compensation: Salary will be negotiated based on previous experience and qualifications.
