CONSULTANT CARDIOLOGIST or
CONSULTANT CARDIOLOGIST and GENERAL MEDICINE
Full Time – 10 Programmed Activities per week
Salary Scale - £ 76,761 - £ 103,490 per annum
Applications are invited for an enthusiastic individual to join the Cardiology team in Forth Valley. The

The Cardiology Department and General medical Services at NHS Forth Valley aim to provide safe,

The post holder will replace sessions released by colleagues pursuing alternative local and regional
duties and will work within the existing team of 5 Consultant Cardiologists. The development of a
subspecialty interest will be encouraged and supported. In particular, the department would look
favourably on a Consultant cardiologist with an interest in pacing, heart failure +/- devices and
cardiac MRI. Candidates will be expected to have a wide range of experience in Cardiology and
possess the MRCP (UK) or equivalent. Candidates with additional accreditation in General Medicine
would be considered favourably.
The Cardiology service functions as an integrated unit with all members contributing to a teambased approach to patient care and this appointment will play an important role in the development
and continuous quality improvement of Cardiology services. The Cardiology senior team comprises
five Consultants, one Consultant Nurse/Cardiology Department Manager and a Cardiology
Investigations Manager. The department is supported by 1 Specialty Doctor, 3 Advanced Nurse
Practitioners (Heart Failure) and a multidisciplinary Cardiac Rehabilitation Team including
Physiotherapy and 8 Specialist Nurses. We also have a team of Specialist and Highly Specialist
Clinical Physiologists (EHRA and BSE accredited).
NHS Forth Valley provides acute services for a population of 310,000 people in Central Scotland.
The new Forth Valley Royal Hospital became fully operational in July 2011. The £300m facility is not true now with the new Southern General in Glasgow one of the most modern and well-equipped
hospitals in Europe. The Cardiology Department is purpose built and includes an integrated outpatient suite hosting facilities for Cardiac Investigations, including echocardiography, exercise
tolerance testing and rhythm management. It is adjacent to a 14 bedded Cardiology Ward and a
Chest Pain Assessment Unit accommodating 6 patients. Additional Cardiology beds, particularly
identified for patients with Heart Failure, are hosted in Ward A31 and the in-patient heart failure
service is led by our team of heart failure nurses.

There will be a contribution to acute general medical receiving for those candidates with
accreditation in General Medicine, by negotiation.The General Medical receiving model has 1
Consultant available in the Acute Admissions Unit from 8am until 8 30 pm and a second Consultant
available at different times to support the emergency take .There are currently 25 Consultants on
the General medical rota.

OVERVIEW OF THE DEPARTMENT AND POST
General Medicine and Acute Medical Receiving
Since the centralisation of the emergency services, there has been an average of 37 emergency
admissions per day (of all ages) with 25 patients admitted under general medicine. There is a 16bedded Combined Assessment Unit (CAU) shared with surgical specialties and adjacent to the A&E
Department. There is a 36-bedded Acute Admissions Unit adjacent to CAU. Patients admitted to
hospital from AAU and triaged to the relevant specialty specific clinical area (critical care,
cardiology, gastroenterology, respiratory, endocrine, stroke, general medicine or ageing and health.
There is a large Day Medicine unit on the FVRH site which provides a range of options for
ambulatory management of patients.
Patients likely to be discharged within 48 hours of admission are discharged from the Acute
Admissions Unit by either the Acute Medicine team (5 existing Consultant acute physicians) or the
on call team responsible for the admission. Patients likely to stay longer will be transferred to a
specialist inpatient ward. Those patients requiring longer periods of rehabilitation are transferred to
the Community Hospital facilities under the care of colleagues in Ageing and Health medicine.
There are 25 Consultant Physicians on the Acute Medical Receiving Rota, with a 1:24.5 on call
overnight frequency and participation in General Medical receiving 1:12.. The process of refining
acute medical care will evolve over the coming months.

Acute Medicine:
The Acute Medicine service has at present 5 substantive Consultants in post: Dr David Thetford,
Dr Dan Beckett, Dr Leah Leitch, Dr Catherine MacLean and Dr Jennifer Gray.

Cardiology
There are currently 3 Consultant Physicians with a special interest in Cardiology: These are Dr.
Allan Bridges, Dr Stephen Glen and Dr Alistair Hargreaves who contribute to the acute medicine
on call rota. Dr James Spratt and Dr Catherine Labinjoh are Consultant Cardiologists with no
commitment to acute medicine. Specialty Doctor – Dr Linda McShane.

Diabetes / Endocrinology
There are six Consultant Physicians with a special interest in Diabetes and Endocrinology: Dr
Chris Kelly, Dr Nick Barwell, Dr John Doig, Dr Linda Buchanan and Dr Alison MacKenzie and Dr
Laura McLaren

Gastroenterology
There are eight Consultants Physicians with an interest in Gastroenterology: Dr David Watts, Dr
Peter Bramley, Dr Richard Burnham, Dr Stuart Paterson, Dr Tim Herron, Dr Tim Gordon-Walker,
Dr Santosh Salunke and Dr Sandeep Siddhi

Respiratory Medicine

There is further support from a group of 9 Consultant Physicians in Ageing & Health, 3
Consultant Haematologists who look after inpatients with haematological and lymphoproliferative
disorders, 3 Consultant Rheumatologists and an Associate Specialist in Rheumatology, 2.5
Consultant Neurologists and 1 Consultant in Rehabilitation Medicine. There are visiting
Consultants in Nephrology and Oncology.

Outpatient Services
Consultant Cardiologists see a combination of general cardiology and specialty patients. A
cardiac genetics service is provided by Dr Ruth McGowan with a monthly cardiac genetics clinic.
There are also a host of multidisciplinary and nurse led out-patients services that are supported
by the Consultant Nurse and Consultant Cardiologists with clinics in Heart Failure, Coronary
Intervention Follow-Up, Ischaemic Heart Disease, Atrial Fibrillation, Aortic Valve Disease and
Suspected Angina.
Suspected Angina Assessment Service –There are 6 clinics per week. Patients with suspected
angina are offered a prompt appointment. The clinic is run by a Specialist Nurse and Senior
Clinical Physiologist. The clinic is also supported by the Cardiologist of the Week.
Specialist Valve Clinic - 2 clinic sessions per week for patients with mild to moderate Aortic Valve
disease or aortic valve prostheses who require ongoing surveillance. The clinic is run by a
Specialist Nurse and Highly Specialist Clinical Physiologist and supported by a Consultant
Cardiologist.
Post Coronary Intervention Clinic – There are 2 clinics per week. Patients post-coronary
intervention are reviewed by a Specialist Nurse. Both pharmacological and non-pharmacological
aspects of care addressed.
Heart Failure Service - Patients are reviewed by the Heart Failure Nursing Team whilst inpatients and followed up in the out-patient clinic (7 sessions). There is also provision for patients
requiring outreach and palliative care.
Ischaemic Heart Disease Clinic – A nurse-led service offering rapid out-patient assessment of
patients with known ischaemic heart disease and deteriorating symptoms.
Investigations
Electrocardiography, ambulatory electrocardiography including event monitors.
Ambulatory blood pressure monitoring.
Exercise Tolerance Testing.
Echocardiography and Specialist Echocardiography including Transoesophageal
echocardiography, Exercise and Dobutamine Stress echocardiography and Contrast
echocardiography.
CT Coronary Angiography (Limited service).
Coronary Angiography and percutaneous coronary intervention performed on either the
Edinburgh Royal Infirmary or Golden Jubilee National Hospital sites.

Devices
Permanent Pacemakers are implanted at Edinburgh Royal Infirmary.
ICDs are implanted on site (FVRH) and in Edinburgh Royal Infirmary.
The successful applicant will be expected to participate actively in the day to day working of the
Cardiology service as well as supporting service development and continuous quality improvement.
They will be required to work closely with Consultant colleagues and the wider Cardiology multidisciplinary team. The focus will be General Cardiology, however, the development of a
subspecialty interest will be encouraged and supported. Forth Valley has an award-winning Heart
Failure service and our team would particularly value the addition of a clinician with an interest in
Heart Failure to drive forward and build on our success in Heart Failure service provision. Additional
skills in pacemaker, cardiac device implantation and/or cardiac MR imaging would be viewed
positively. Specialist accreditation in General Medicine is desirable and applicants with this
qualification would be viewed favourably.
Forth Valley NHS aims to deliver high quality, patient-centred care and we aim to provide outpatient
services in a timely fashion and in accordance with current waiting time directives. It is expected that
the successful candidate will provide two general cardiology outpatient clinics per week plus a
specialty outpatient clinic (minimum of 126 clinics per year although this along with other aspects of
the job plan will be dependent on any potential general medical component) and support the work of
nurse-led outpatient services.
There is no Cardiology out-of-hours service in FVRH with patients being cared for, in the first
instance, by the general physician on call. Support, where required, comes from our tertiary service
providers with whom we have service level agreements (NHS Lothian and NHS Greater Glasgow
and Clyde) and is predominantly but not exclusively concerned with the provision of primary PCI
services. It therefore vital that in-patients are cared for with clear, up to date management plans at
all times and good communication with tertiary centres where required. NHS Forth Valley
Consultant Cardiologists provide an on call service during working hours for one week in every five
with prospective cover, accepting new inpatients and supporting the wider hospital teams as well as
local general practitioners.
Postgraduate and Developmental Facilities
Forth Valley Royal Hospital has a Postgraduate Centre with Lecture Theatre and modern high
quality audio-visual facilities and a well equipped medical library. It offers opportunities for a wide
range of development activities for all staff groups.
A wide range of full text and e-journals are freely available. Internet access is available in the centre
and also in Consultant accommodation.
There are regular postgraduate meetings with telemedicine links to Glasgow and Edinburgh with
their Royal Colleges. University medical schools and postgraduate facilities are within easy reach.
Forth Valley Royal Hospital is the site of the Scottish Centre for Simulation and Clinical Human
Factors.

CONSULTANT CARDIOLOGIST RESPONSIBILITIES

As agreed with the General Manager and Associate Medical Director (Medical Directorate)
to provide with Consultant colleagues an area wide service with responsibility for the
prevention, diagnosis and management of illness, and for the proper functioning of the
service;

To provide cover for Consultant colleagues during annual and study leave or at such other
times as agreed with the General Manager and Associate Medical Director ;

To offer professional supervision, training and appraisal of junior medical, nursing and
technical staff as appropriate to enhance role extension and job skills within the service in
order to improve the service provided to patients;

To motivate staff within the service, through leading by example, and fostering good working
relationships at all levels in line with the principles of the local Partnership Agreements;

To contribute to developments and initiatives within the organisation as appropriate and as
requested;

To undertake teaching, accreditation and examination duties, and contribute to postgraduate
and continuing medical education activity across the area service;

To agree and implement an annual medical audit and clinical governance programme with
the Associate Medical Director in line with the Board’s Clinical Effectiveness Strategy;

To participate fully in Consultant appraisal and personal development planning activities;

To ensure the efficient and effective use of the organisation’s resources;

To work with colleagues to ensure the effective and efficient development and
implementation of support systems.
The requirements of this post will change through time in line with clinical advances and
developments in services provided by the NHS in Forth Valley.

CONTRACTUAL DETAILS
Contracted activities for this appointment will be defined and agreed with the post holder and the
General Manager, Medical Directorate.
The post holder will be professionally responsible to the Associate Medical Director, Dr Allan
Bridges and managerially responsible to the General Manager, Mr Ian Aitken
The Consultants in the department will agree arrangements for cover of cardiology duties.
This Consultant post is available on a full-time basis but the Board welcomes applications from
individuals who are unable to undertake a full time commitment and who may wish to work on either
a part-time or job share basis.
The terms and conditions of service applicable to this appointment are the Consultant grade terms
and conditions of service as amended from time to time.
NHS Forth Valley will require each Consultant to agree with the General Manager and Associate
Medical Director, a job plan of fixed and other commitments, which will be reviewed on an annual
basis and revised as necessary to meet the requirements of the local and national service issues.
Consultants will be required to participate in an annual appraisal.
The post holder will normally be expected to reside within 30 minutes travelling distance of Forth
Valley Royal hospital. If NHS Forth Valley Health Board considers that the there is a requirement
for the post holder to relocate his or her home in order to take up the responsibilities of the post,
relocation expenses will be available in accordance with the Board’s Relocation Policy. Support to
the level of 15% of starting salary up to a maximum of £10,000 may be available, subject to the
provisions of the policy.
Formal appointment to this post will be conditional upon satisfactory completion of all preemployment checks, including satisfactory medical screening.
NHS Forth Valley does not negotiate salary placements. On commencement of employment,
salary will be determined in accordance with Section 5 of the terms and conditions of
service.
This appointment will be superannuable under the NHS Superannuation Scheme, which is
contracted out of the State Earnings Related Pension Scheme (SERPS). New employees will

The employment is subject to three calendar months notice on either side.
The successful candidate will be expected to be registered for CPD with the Royal College of
Physicians and to fulfil requirements for annual certification and for appropriate revalidation.
Annual leave entitlement is six weeks per year (pro rata for part time post holders), with additional
statutory holiday entitlement as agreed by the Local Negotiating Committee and published annually.

JOB PLAN
The post holder is required to agree a detailed job plan which will be reviewed at least annually.
Efforts will be made to complete job planning prior to the date of commencement in post, or, if this is
not possible, as soon as possible following this date.
The detail of fixed commitments, including any special interest, will be finalised following
appointment of the successful applicant. This is a 10 Programmed Activity post of which one SPA
will be allocated for Personal Development.
Following commencement in post, depending on the needs of the service and the particular
attributes of the post holder, it may be possible to negotiate additional time for Supporting
Professional Activities to include audit, teaching and training. This will require the approval of the
General Manager and Associate Medical Director.
Supporting professional activities will include all of the Consultant’s professional duties and
commitment. The exact timing and location of the Supporting Professional Activities will be agreed
during the job plan review with the General Manager and Associate Medical Director and included in
the prospective job plan.

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme
The Protecting Vulnerable Groups (PVG) scheme has replaced previous disclosure arrangements for
people who work with vulnerable groups. The duties of this post require will require the post holder to
undertake regulated work with children and young people (individuals aged under 18 years) and / or
protected adults (individuals aged 16 or over who is provided with (and thus receives) a type of care,
support or welfare service).
The post holder will be required to be a member of the PVG Scheme and to consent to NHS Forth
