This entitles a club member to enter the Club Ballot for Wimbledon tickets and increases the number of pairs of tickets allocated to the club. British Tennis Membership is free if you are a member of an LTA affiliated club, so if you have not already done so please register online www.lta.org.uk or tel 0845 8737202. This year our allocation will be based on the number of club members who were already members of British Tennis and who ‘opted-in’ to the club ballot by 15 Feb 2015. If you are a BTM do you want to be entered into the club ballot (to be held during April/May) Name: ……………………………………………………….. Yes / No Note: The following rules apply: 1. Only a paid up member (full or affiliate and over 8 years old*) who is also a British Tennis Member (BTM) may enter the ballot (*LTA Ruling). The BTM must also ‘opt-in’ to the ballot. 2. Winners will receive a pair of tickets which they must pay for 3. The member who has won must go - tickets cannot be passed to a third party. The club believes that it is important that members, coaches, administrators and parents/guardians associated with the club should, at all times, show respect and understanding for the safety and welfare of others. Therefore, members are encouraged to be open at all times and to share any concerns or complaints that they may have about any aspect of the club with the Child Protection Officer or a member of the Committee As the parent/guardian of a member of Ottery St Mary Tennis Club you are expected to abide by the following code of practice: • • • • • • • • • • • • • • Encourage your child to learn the rules of tennis and play within them. Discourage unfair play and arguing with officials. Help your child to recognise good performance, not just results. Never force your child to take part in sport. Set a good example by recognising fair play and applauding the good performances of all. Never punish or belittle a child for losing or making mistakes. Publicly accept judgements made by officials. Support your child’s involvement and help them to enjoy their sport. Use correct and proper language at all times. Allow your child to play in the Mini Tennis colour stage that is appropriate to them. This will ensure they enjoy playing the game, develop their skills and gain confidence. Purchase balls and rackets that match the stage your child is in. Be patient. Steady progression is unusual in children; peaks and plateaus are common. Your first question following any match should be: “Did you enjoy it?” not “Did you win?” At no stage should you communicate with your child or interfere with the on-court helpers and referees during a match – just enjoy the game and let officials take care of the rules Parent/guardian declaration By signing and returning this form, I agree to my son/daughter taking part in the general activities of the club. He/she has agreed to follow the junior rules of the club, and I agree to accept the code of conduct for parents (attached) To my knowledge, he/she has no special care needs, dietary requirements, allergies or medical conditions that could affect his/her safety at the club, other than those declared on this form. I understand that in the event of any injury, illness or other medical need, all reasonable steps will be taken to contact me, and to deal with the situation appropriately. I understand that I must inform the club of any changes to the information provided on this form. Signed: …………………………………………………………… .Date:…………………..
