Introductions were made by Ms. Davis, the LEA . She provided Ms. Smith with a copy of the district’s procedural safeguards, briefly reviewed these, and asked if Ms. Smith had any questions. The purpose of the meeting is to conduct an annual review of
John’s progress in the current program and develop a new IEP with updated present levels of performance and annual goals. The team reviewed the draft IEP and
John’s
progress this year. John continues to struggle in ELA. He faces difficulty when reading passages. John uses his time in resource room efficiently and works towards his goals. The team discussed
John’s
participation in the general education setting.
John works in cooperative groups and also independently as requested by the teacher. He has shown progress in working with his peers and participating in classroom discussions. John uses the highlighter to mark important points as an accommodation in the general education classroom.
Ms. Compton , the special education teacher, reported that John has made progress this year and has met his annual goals, but continues to struggle with all tasks that require independent reading of grade level material. The team discussed increasing John’s time in resource in order for him to receive additional instruction in reading. The team agreed to add an additional 30 minutes per week to John’s IEP and developed an additional goal in the area of reading comprehension. John has achieved all his speech goals. New speech goals were developed for next year.
Or if the parent did not attend: Ms. Compton will send the completed IEP, minutes, and a copy of the procedural safeguards to the parent within 7 calendar days of the date of the IEP meeting.

As a result of the IEP/Staffing meeting, the IEP/Staffing Team proposes or refuses (circle one) to:
The IEP team reviewed John’s progress toward his annual goals and current program. A new IEP was developed with an update of John’s PLAAFP and appropriate goals. The team decided to increase amount of special education services.

John achieved his annual goals, but continues to struggle with any task that requires independent reading of grade-level material.

Description of each evaluation procedure, assessment, record, or report the school district used in deciding to propose or refuse the action:
The team reviewed John ’s previous levels of academic achievement and functional performance from the current IEP, his levels of academic achievement and functional performance as updated on 5/10/2013 , his current grades, and input from his teachers and parents.

Description of any other choices that the IEP team considered and the reasons why those choices were rejected:
The IEP team considered maintaining John
’s current level of special education resource assistance.
Discussion by the team of his current reading level and needs in the general classroom clearly demonstrated the need for increasing the previous IEP’s level of support and service time.

Description of any other reasons why the school district proposed or refused (circle one) the action:
This annual review was held in order to comply with state and federal regulations in order to determine whether the annual goals for the student were being met and to revise the IEP to address any lack of expected process toward the goals and in the general education curriculum.
If parent attended, provide a copy of this form and copy of the IEP to the parents.
If parent did not attend, send parent a copy of this form and IEP to the parents.
