This application form is to be used by:

Students who are in the process of enrolling or have recently enrolled in a training
programme which has still not been considered eligible for the scheme.

Education providers that would like the course of studies to be listed in the list of approved
courses for tax credits under the Get Qualified 2014 scheme. In this case, this application
should be followed by at least one (1) student application within twelve (12) months from
the approval issued by Malta Enterprise. Should no applications be handed in within the
stipulated time-frame, the Corporation reserves the right to remove the course from the list
of approved courses and to reject any re-submission of the same certification or
qualification within the following twenty-four (24) months.

2. This Application Form and any attached documents will be treated as confidential
throughout and after the evaluation process.
3. Only Application Forms that include all relevant supporting documents will be evaluated.
4. All entries in this application must be clearly explained and substantiated.
5. Only typed application forms will be accepted. Should the application be hand-written, or
should the format of the application form be altered, the application form will not be
evaluated.

Please note that in case of students filling in the application form:
1. Students, who have applied for funding through any other Government of Malta initiative
for funding costs related to the attainment of a certification, may only apply for a tax credit
in relation to that certification, if such funding is declined.
2. The application forms (together with its annexes) are to reach Malta Enterprise duly
signed, within twelve (12) months from commencement date of course.
3. A copy of Proof of Payments (en cashed cheques, bank transfers or signed and stamped receipt
by the respective institute/awarding body) needs to be forwarded to Malta Enterprise within
three (3) months from successful termination of the course.

4. No Certificates will be issued by the Corporation prior to receiving the requested Proof of
Payments within the stipulated deadline.
Personal Data Protection
The information provided in this form will be processed by Malta Enterprise Corporation to assess
your eligibility, approve or otherwise assistance under the Get Qualified scheme. Where applicable,
the information may also be disclosed and shared with the Inland Revenue Department,
Employment Training Corporation (ETC), Education Department, and any other Governmental Entity
for the purpose of verifying abuse, generating statistical information and any other reason relevant
to ensuring adequate control of public finances. Personal information collected in this form shall be
processed in accordance with the Data Protection Act, Cap 440 of the Laws of Malta. The applicant
has the right to access, rectify, and the right to erase personal data from the application. Malta
Enterprise Corporation guarantees fair processing in respect of the applicant’s personal information.
Furthermore, Malta Enterprise will solely send the applicant’s information related to this application.

I, the undersigned hereby declare that the information being submitted with regards to this
application is correct.
I confirm that the institute adheres to national standards and is operating from licensed premises.
Unless exempted by L.N. 296 of 2012, it is understood that Malta Enterprise shall remove the
provider’s name from the list of providers until the relevant authorities confirms that the provider
has regularised the institute’s position.
By signing this declaration I hereby authorise Malta Enterprise Corporation to process the data
contained in this form for the purpose stated. I also authorise the Corporation, as the administrator

reimbursable from other sources or otherwise recoverable.
It is understood that any aid to be granted by Malta Enterprise is conditional to the
completion of the study course.

data contained in this form for the purpose stated. I also authorise Malta Enterprise

7. Required Documentation (kindly tick where appropriate)
Official documentation showing that the qualification / certification is recognised by
the Malta Qualifications Framework (MQF/EQF) as equivalent to level 5 or higher.4
Official documentation showing the number of ECTS of the Certification from one (1)
of the following :
 List of Courses offered by Licensed Institution provided by NCFHE; or
 Ofqual; or
 Awarding Body
Official documentation issued by the Awarding Body detailing the topics included in
the Course of Studies and the assessments that the student will be expected to
undertake.
If the student shall incur costs from an education provider other than the Awarding
Body:
 An official document from the Awarding Body which certifies that the
provider of these services is a recognised representative.
 If the education provider is also offering training, tuition or other similar
services leading to the attainment of the qualification, the documentation
submitted must also clearly state that the Awarding Body has authorised the
education provider to offer such services.
 The documentation should clearly indicate the commencement and
termination dates of the agreement between the education provider and
the Awarding Body.
 Official documentation issued by the institute showing the total costs of the
course, examination fees, and the cost of one (1) examination/assignment
resit (if applicable)

universities/institutions which may be downloaded from the National Commission for Further and Higher Education’s website.
