However the younger and disabled patients did not make the effort to attend. We will continue to have a larger age spectrum for our group. Are there any specific characteristics of your practice population which means that other groups should be included in the PPG? e.g. a large student population, significant number of jobseekers, large numbers of nursing homes, or a LGBT community? Review of patient feedback Outline the sources of feedback that were reviewed during the year: We have been looking at Did not attend (DNA’s) rates Building Community Relationships Parking issues How frequently were these reviewed with the PRG? Twice yearly 3. Action plan priority areas and implementation Priority area 1 Description of priority area: DNA’s (Did not attend figures) The PPG agreed to evaluate any DNA’s within the practice should be highlighted is some form or other. This has never been done at the surgery till now What actions were taken to address the priority? An audit was completed on the past four months Result of actions and impact on patients and carers (including how publicised): DNA’s- 294 DNA’s for a four month period. Averaging approximately 70 patients per month. This is an unacceptable amount which blocks valuable consultations for patients. When patients don’t turn up it’s difficult for GPs to use time constructively as they have to keep an eye out for if the patient turns up late so keeping the need to check the appointments screen. Just as frustrating is patients who arrive late, particularly if its 5-10 minutes late, especially the repeat offenders, this then makes the rest of the surgery run late for other patients and staff. We will be making these statistics available in the waiting areas and looking at possibly using text messages to remind patients before appointments. Priority area 2 Description of priority area: Building Community Relationships What actions were taken to address the priority? Val who we employ as a Care Co-ordinator (CCC) continues to network locally within the community raising awareness of services and support available e.g. Community Drop In sessions Result of actions and impact on patients and carers (including how publicised): Relationships and services created: a) Drop in Service at Millington Close Baschurch-Looking at Housing issues,debt,benefits etc.These are done via support of Shropshire Towns and Rural Housing b) Macmillan coffee morning events-Raising awareness of community and funds involvement c)Awareness Event at Millington Close-Outside organisations invited for Information stands e.g. Red Cross,Mears,AgeUK in attendance. Priority area 3 Description of priority area: Parking Issues What actions were taken to address the priority? Have asked for permission to one of the patients who live close by and has available parking at her home. This would be used to accommodate staff parking. There is a local move for a parking area to be made available off site close by to be used for staff . Planning permission has been requested and we are awaiting the outcome. Result of actions and impact on patients and carers (including how publicised): Due to the limited availability of present parking staff take up quite a large proportion of the available spaces. Also we have visiting services like district nurses, counselling ,diabetic eye screening and foot screening which also take up space. Progress on previous years If you have participated in this scheme for more than one year, outline progress made on issues raised in the previous year(s): Even though the PPG have not grown to the extent we would like their input and suggestions have been invaluable. The PPG members had a look around the waiting area and suggested that we remove the many magazines that were lying around and actually cluttering the area. The PPG also suggested that the information on the notice boards also be de-cluttered and the information be made more up to date. This was all looked at and acted upon and we updated all our notice boards. We found that there was definitely an increase in readership. Any suggestions and comments were acted upon e.g. one of which was a complaint that our “zero tolerance” sign was not appropriate, we then created a more streamlined wording to have the same effect but less threatening for patients to read. Parking has been a continuing issue and we have asked that staff park at one of our patient’s houses who had offered. This has not resolved the issue but has helped. 4. PPG Sign Off Report signed off by PPG: YES Date of sign off: 26.02.2015 How has the practice engaged with the PPG: How has the practice made efforts to engage with seldom heard groups in the practice population? Has the practice received patient and carer feedback from a variety of sources? Was the PPG involved in the agreement of priority areas and the resulting action plan? How has the service offered to patients and carers improved as a result of the implementation of the action plan? Do you have any other comments about the PPG or practice in relation to this area of work? We have tried to involve and invite younger groups and disabled patients to our meetings. There is always an initial positive outcome but we find that due to valid excuses the new members do not come to the meetings. We will however carry on trying. We get feedback from especially carers on how well the GPs and DNs have looked after their loved ones. Usually we only get the feedback once the patient has passed away and at Christmas time. The PPG have been involved in the priority areas and implementation of the action plans. Even though there have not been many action plans, the PPG have been instrumental in making and suggesting changes that we would have possibly not looked at in finer detail if it wasn’t for their input and suggestions. Have enclosed minutes which show the progress of the changes and discussions held.
