Risk Assessment Record
Site: Worcester Road Filling Station
Contract No......................................................................... Date............................................
Activity: GAS MONITORING
A Clearance Certificate must be completed for each task each day. The contents of the clearance certificate and this document must be discussed fully with the site
representative ( where appropriate) and the work team.
METHODS AND SEQUENCES
Understanding and acceptance must be recorded on the last page of this document.
Critical tasks completed by (signature)
1 Check gas monitor is in calibration and has appropriate sensors for the risk i.e ) O 2 CO H2S & LEL
2 Check function of all sensors by taking a fresh air calibration in *clean* breathable air
3 Ensure that the monitors battery has enough power to last for the entire duration of the task being monitored
4 Slowly lower the monitor into the area being monitored to within 300mm of the base avoiding bodily entry
5 Where the monitor may recover on being removed, set on maximum mode to record the worst case concentrations
6 Do not allow silicone products to contaminate the sensor apertures
7 Do not test sensors by placing in heavily contaminated atmospheres as this will poison the toxic sensors
8 Always follow manufacturer’s instructions for use and care.
9 Flammable atmospheres are categorised as:
10
Zone 0 – Flammable atmosphere constantly present.
Zone 1 – Flammable atmosphere likely in normal operation.
Zone 2 – Flammable atmosphere may exist for short periods.
11 LEL readings must be <10% LEL for cold works and< 1% LEL for hot works
12 Toxic readings must be < 50% of WEL
13 Oxygen reading must be between 19% and 23%
